摘要
目的探讨二陈汤合三子养亲汤在慢性阻塞性肺疾病(慢阻肺)急性加重期(AECOPD)痰湿壅肺证治疗中的应用效果。方法84例AECOPD痰湿壅肺证患者,采用随机数字表法分为对照组和观察组,每组42例。对照组采用常规西药治疗,观察组在对照组基础上采用二陈汤合三子养亲汤治疗。比较两组治疗效果、临床症状消失时间及治疗前后超敏C反应蛋白(hs-CRP)、中医证候积分、白细胞计数(WBC)。结果观察组治疗总有效率为97.62%(41/42),高于对照组的80.95%(34/42),差异具有统计学意义(P<0.05)。治疗后,观察组hs-CRP、中医证候积分和WBC分别为(4.17±1.28)mg/L、(0.54±0.10)分、(7.17±1.28)×10^(9)/L,低于对照组的(8.20±2.43)mg/L、(1.50±0.52)分、(9.23±1.65)×10^(9)/L,差异具有统计学意义(P<0.05)。观察组咳嗽消失时间(7.08±1.21)d、咳痰消失时间(5.74±0.75)d、喘息消失时间(5.09±1.40)d短于对照组的(9.14±1.77)、(7.87±0.76)、(7.10±1.19)d,差异具有统计学意义(P<0.05)。结论二陈汤合三子养亲汤在AECOPD痰湿壅肺证治疗中的应用价值较高,不仅可以及早缓解临床症状,促进治疗效果的提高,而且能够改善患者WBC与hs-CRP水平,值得临床采纳与推广。
Objective To discuss the effects of applying Erchen decoction combined with Sanzi Yangqin decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with phlegm dampness obstructing lung syndrome.Methods 84 patients with AECOPD with phlegm dampness obstructing lung syndrome were randomly divided into control group and observation group,with 42 cases in each group.Patients in the control group received conventional western medicine,and patients in the observation group received Erchen decoction combined with Sanzi Yangqin decoction in addition to the treatment to the control group.The therapeutic effects,disappearance time of clinical symptoms,high-sensitivity C-reactive protein(hs-CRP),traditional Chinese medicine syndrome score and white blood cell(WBC)count before and after the treatment were compared between the two groups.Results The overall efficacy of the observation group was 97.62%(41/42),which was higher than 80.95%(34/42)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the observation group had hs-CRP of(4.17±1.28)mg/L,traditional Chinese medicine syndrome score of(0.54±0.10)points and WBC of(7.17±1.28)×10^(9)/L,which were lower than(8.20±2.43)mg/L,(1.50±0.52)points,(9.23±1.65)×10^(9)/L of the control group,respectively;and the differences were statistically significant(P<0.05).The observation group had cough disappearance time of(7.08±1.21)d,expectoration disappearance time of(5.74±0.75)d and wheezing disappearance time of(5.09±1.40)d,which were shorter than(9.14±1.77),(7.87±0.76),(7.10±1.19)d of the control group;and the differences were statistically significant(P<0.05).Conclusion Erchen decoction combined with Sanzi Yangqin decoction has high application value in the treatment of AECOPD with phlegm dampness obstructing lung syndrome,which can not only relieve symptoms and improve the efficacy,but also improve the levels of WBC and CRP in patients.Thus,it is worthy of promotion.
