摘要
目的:比较早期异位妊娠(EP)与早期妊娠患者血清中血管内皮生长因子(VEGF)、促血管生成素-1(Ang-1)和促血管生成素-2(Ang-2)的水平差异,从而确定其在早期异位妊娠诊断中的价值。方法:选取53例最终确诊为异位妊娠患者为研究对象(异位妊娠组),选择同期50例正常宫内早期妊娠患者为对照(正常早孕组),采用ELISA法检测两组患者血清中VEGF、Ang-1和Ang-2浓度。结果:与正常早孕组比较,异位妊娠组VEGF浓度明显升高,Ang-2浓度明显降低,差异具有统计学意义(P<0.05)。VEGF单独检测的受试者工作特征(ROC)曲线下面积(AUC)为0.889(0.812~0.942),灵敏度为77.4%,特异度为92%,阳性预测值(PPV)为89.1%,阴性预测值(NPV)为78.9%。Ang-2单独检测的AUC、灵敏度、特异度、PPV、NPV分别为0.854、79.2%、80.0%、80.8%、78.4%。VEGF和Ang-2联合检测的AUC、灵敏度、特异度、PPV、NPV分别为0.916、81.1%、92.0%、91.5%、82.1%。联合检测与VEGF或Ang-2单独检测的AUC无统计学差异。结论:血清VEGF和Ang-2单独检测对EP的早期诊断均具有较高的灵敏度和特异度,联合检测未能显著提高其诊断效力。
Objective: To compare the serum levels of vascular endothelial growth factor(VEGF), angiopoietin-1(Ang-1), and angiopoietin 2(Ang-2) in early ectopic pregnancy and investigate their diagnostic value. Methods: Fifty-three patients finally diagnosed with ectopic pregnancy were selected as the study subjects(ectopic pregnancy group), 50 patients with normal pregnancy were selected as the controls(normal pregnancy group), and their serum levels of VEGF, Ang-1, and Ang-2 were detected by ELISA. Results: Compared with normal pregnancy group, concentration of VEGF was significantly increased but Ang-2 was significantly decreased in ectopic pregnancy group(P<0. 05). The area under the receiver operator characteristic(ROC) curve(AUC) of VEGF was 0. 889(0. 812-0. 942),the sensitivity was 77. 4%, the specificity was 92%, the positive predictive value(PPV) was 89. 1%,and the negative predictive value(NPV) was 78. 9%. The AUC, sensitivity, specificity, PPV and NPV of Ang-2 were 0. 854, 79. 2%, 80. 0%, 80. 8%, and 78. 4%, respectively. The AUC, sensitivity, specificity, PPV and NPV of combined VEGF and Ang-2 were 0. 916, 81. 1%, 92. 0%,91. 5%,and 82. 1%, respectively. There was no statistically significant difference of AUC between combined detection and single detection of VEGF and Ang-2. Conclusion: Serum VEGF or Ang-2 detection has high sensitivity and specificity in diagnozing ectopic pregnancy, while the combined detection of VEGF and Ang-2 does not significantly improve the diagnostic efficacy.
