摘要
目的:探析骨水泥型加长柄人工髋关节置换术治疗高龄股骨粗隆间骨折患者的临床疗效。方法:回顾性分析2018年8月~2020年8月期间收治的50例高龄股骨粗隆间骨折患者各项临床资料,按自愿入组将其分为对照组和研究组,每组25例。对照组采用常规PFNA内固定的方法治疗,研究组采用骨水泥型加长柄髋关节置换治疗,记录两组出血量、输血量、平均下床负重行走时间、血红蛋白、疼痛程度。结果:研究组出血量、输血量多于对照组,但术后平均下床负重行走时间短于对照组,差异有统计学意义(P<0.05)。术前,两组血红蛋白水平基本一致,差异无统计学意义(P>0.05);术后,两组血红蛋白水平均已下降,研究组降幅小于对照组,差异有统计学意义(P<0.05)。术后7 d与14 d,研究组VAS评分均低于对照组。结论:高龄股骨粗隆间骨折患者行骨水泥型加长柄人工髋关节置换术治疗,虽然出血量与输血量多,但术后疼痛轻,能够缩短患者术后下床时间。
Objective To explore the clinical effect of bone cement lengthened handle hip arthroplasty for elderly patients with intertrochanteric fracture.Method The clinical data of 50 elderly patients with intertrochanteric fracture of femur admitted to our hospital from August 2018 to August 2020 were retrospectively analyzed,and they were divided into control group and study group according to voluntary admission,with 25 cases in each group.The control group was treated with conventional PFNA internal fixation,and the research group was treated with bone cement type elongated handle hip arthroplasty.The blood loss,blood transfusion,average walking time out of bed,hemoglobin,and pain degree were recorded in both groups.Results The amount of blood loss and blood transfusion in the study group was higher than that in the control group,but the average time of weight-bearing walking after operation was shorter than that in the control group(P<0.05).Before operation,the hemoglobin level of the two groups was basically the same(P>0.05).After surgery,hemoglobin levels in both groups decreased,but the decrease in the study group was less than that in the control group(P<0.05).The VAS score of the study group was lower than that of the control group at 7 d and 14 d after surgery.Conclusion In elderly patients with intertrochanteric fracture of femur,bone cement type elongated handle arthroplasty can shorten the postoperative time of patients getting out of bed,although the amount of blood loss and blood transfusion is large,but the postoperative pain is light.
