摘要
目的:探讨磁共振弥散加权成像(DWI)及表观扩散系数(ADC)在肾积水肾功能评估中的作用。方法:回顾性分析2018年5月~2020年6月的176例单侧肾积水患者作为观察组,另选同期于本院行健康体检的176例正常居民为对照组,两组均行MRI检查,测量两组肾脏的ADC值,并记录两组的肾小球滤过率(GFR)和血清肌酐清除率(CCr)等肾功能指标情况。采用Pearson法对观察组各参数的相关性进行分析。结果:观察组肾脏的ADC值明显低于对照组,差异有统计学意义(P<0.05)。观察组的GFR值明显低于对照组,而CCr则明显高于对照组,差异均有统计学意义(P<0.05)。重度肾积水患者的ADC值明显高于轻中度患者,差异有统计学意义(P<0.05)。Pearson相关分析显示,GFR与ADC值呈明显正相关,CCr与ADC值呈负相关(P<0.05)。结论:磁共振弥散加权成像中ADC值在肾积水患者中异常降低,能较好地反映肾功能状态变化,故可应用于肾积水患者的肾功能评估。
Objective:To investigate the role of diffusion weighted imaging(DWI)and apparent diffusion coefficient(ADC)in the evaluation of renal function in hydronephrosis.Methods:176 patients with unilateral hydronephrosis from May 2018 to June 2020 were retrospectively analyzed as the observation group,and 176 normal residents who underwent physical examination in our hospital in the same period were selected as the control group.MRI was performed in both groups to measure the ADC value of the kidneys of the two groups,and the renal function indexes such as glomerular filtration rate(GFR)and serum creatinine clearance(CCr)were recorded.Pearson method was used to analyze the correlation of various parameters in the observation group.Results:The ADC value of kidney in the observation group was significantly lower than that in the control group(P<0.05).The GFR value of the observation group was significantly lower than that of the control group,while the CCR was significantly higher than that of the control group(P<0.05).The ADC value of children with severe hydronephrosis was significantly higher than that of children with mild and moderate hydronephrosis,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that GFR was positively correlated with ADC value,and CCR was negatively correlated with ADC value(P<0.05).Conclusion:The ADC value in diffusion-weighted magnetic resonance imaging is abnormally reduced in patients with hydronephrosis,which can better reflect the changes of renal function.Therefore,it can be applied to the evaluation of renal function in patients with hydronephrosis.
