磁共振扩散加权成像与动态对比增强在梗阻性肾积水肾功能评价中的应用 被引量:6

The Application of Diffusion Weighted Imaging and Dynamic Contrast-Enhanced MR in Evaluating Renal Function in Patients with Obstructive Hydronephrosis

摘要 目的探讨磁共振扩散加权成像(DWI)和动态对比增强(DCE)在梗阻性肾积水肾功能评价的应用价值。方法对19例单侧梗阻性肾积水患者和13名正常人行磁共振尿路成像(MRU)、DWI和DCE,测量双侧肾脏的表观扩散系数(ADC)值,描绘出肾脏皮质、髓质平均时间-信号强度曲线(TIC),记录皮质、髓质信号峰值,计算出肾脏通过时间(RTT),对各参数之间进行相关性分析。结果积水肾脏ADC值均显著低于正常对照组(P<0.001);积水肾脏与正常肾脏皮质信号峰值差异比较无统计学意义,积水肾脏髓质信号峰值较正常肾脏显著降低(P<0.01);RTT与ADC值呈明显负相关(r=-0.693,P<0.001),髓质信号峰值与ADC值呈明显正相关(r=0.762,P<0.001)。结论 DWI和DCE可以初步评估梗阻性肾积水肾功能改变。 Objective To assess the application value of diffusion weighted imaging( DWI) and dynamic contrast-enhanced MR( DCE-MR) in determining the renal function in patients with obstructive hydronephrosis.Methods A total of19 patients with unilateral hydronephrosis( study group) and 13 healthy subjects( control group) were enrolled in this study.MR urography( MRU),DWI and DCE-MR were performed in all subjects.The ADC values of bilateral kidneys were measured,the mean time-signal intensity curves of both cortex and medulla of bilateral kidneys were delineated,the peak intensity values of cortex and medulla( Pc and Pm) were recorded,and the renal transit time( RTT) was calculated.The correlations between all parameters were analyzed.Results The ADC value in the study group was significantly lower than that in the control group( P〈0.001),No statistically significant difference in Pc value existed between the two groups.Pm value of the study group was significantly lower than that of the control group( P〈0.01).In the study group,a significant negative correlation was found between RTT value and ADC( r =- 0.693,P〈0.001),while a significant positive correlation existed between Pm and ADC( r = 0.762,P〈0.001).Conclusion Diffusion weighted imaging and dynamic contrast-enhanced MR can approximately evaluate the renal function changes in patients with obstructive hydronephrosis.
