摘要
目的研究口服氨甲环酸联合窄谱强脉冲光(DPL)治疗黄褐斑的临床效果和安全性。方法将纳入研究的75例黄褐斑患者随机分为3组,每组25例。药物组仅接受口服氨甲环酸治疗,DPL组仅接受DPL治疗,联合组接受口服氨甲环酸联合DPL治疗。所有入组患者治疗前后均通过黄褐斑皮损面积及严重程度评分(MASI)评估,以治疗前后MASI分值下降率(MDR)为指标进行疗效评定。观察各组患者不良反应及复发情况。结果药物组、DPL组与联合组平均MDR分别为(55.18±23.14)%、(51.41±19.36)%、(70.52±17.25)%,联合组MDR高于药物组及DPL组,差异有统计学意义(P<0.001),3组患者均无明显不良反应。经3~12个月的随访,药物组、联合组复发率均为4.00%(1/25),DPL组复发率为12.50%(3/24)。结论DPL联合氨甲环酸口服治疗黄褐斑安全有效,不良反应发生率和复发率低。
Objective To evaluate the effect and safety of oral tranexamic acid combined with delicate pulsed light(DPL)in the treatment of melasma.Methods A total of 75 patients with melasma included in the study were randomly divided into 3 groups,with 25 cases in each group.Medicine group only received oral tranexamic acidtreatment,DPL group only received DPL treatment,and combination group received oral tranexamic acid combined with DPL treatment.All patients were assessed by melasma area severity index(MASI)before and after treatment,and the rate of decrease in MASI score(MDR)before and after treatment was used as an indicator to evaluate the effect.The adverse reactions and recurrence of each group of patients were observed.Results The average MDR of the medicine group,DPL group and combination group were(55.18±23.14)%,(51.41±19.36)%,(70.52±17.25)%,respectively.The MDR of the combination group was higher than that of the medicine group and DPL group(P<0.001).There were no obvious adverse reactions in the three groups.After 3 to 12 months of follow-up,the recurrence rate of the medicine group and the combination group were 4.00%(1/25),and the recurrence rate of the DPL group was 12.50%(3/24).Conclusion Oral tranexamic acid combined with DPL in the treatment of melasma is safe and effective,with low incidence of adverse reactions and recurrence.
