外周血单个核细胞TFAM、GDF-15、淋巴细胞β1-AR的mRNA对近期MACE的预测效能 被引量:4

摘要 目的探究外周血单个核细胞(PBMCs)线粒体转录因子A(TFAM)、生长分化因子-15(GDF15)、淋巴细胞β1肾上腺素受体(β1-AR)的mRNA对近期主要心血管不良事件(MACE)的预测效能。方法选取延安大学附属医院2017年4月~2019年10月CHF患者127例作为研究对象,分析外周血指标与心肌重构指标[左心房内径(LAP)、左室后壁厚度(LVPW)、左室舒张末内径(LVEDD)、左室质量指数(LVMI)、左室重构指数(LVRI)]、LVEF相关性及对MACE的预测价值。结果PBMCs中TFAM、淋巴细胞β1-AR mRNA相对含量与LAP、LVPW、LVEDD、LVMI呈负相关,与LVRI、LVEF呈正相关,GDF-15 mRNA相对含量与LAP、LVPW、LVEDD、LVMI呈正相关,与LVRI、LVEF呈负相关(P<0.05);随访3个月,发生MACE组PBMCs中TFAM、淋巴细胞β1-AR mRNA相对含量低于未发生MACE组,GDF15 mRNA高于未发生MACE组(P<0.05);PBMCs中TFAM、GDF-15、淋巴细胞β1-AR mRNA相对含量联合预测MACE的AUC值为0.889,大于任一指标单独预测。结论CHF患者PBMCs中TFAM、GDF-15、淋巴细胞β1-AR mRNA表达异常,在其病理变化中发挥重要作用,且表达水平与MACE发生关系密切,可能成为CHF新的标志物及治疗靶点。 Objective To explore the predictive performance of mitochondrial transcription factor A(TFAM),growth differentiation factor-15(GDF-15),lymphocyteβ1 adrenergic receptor(β1-AR)mRNA in peripheral blood mononuclear cells(PBMCs)for occurrence of recent major adverse cardiovascular events(MACE).Methods A total of 127 CHF patients were recruited from the Affiliated Hospital of Yan’an University during April 2017 to October 2019.Predictive value for MACE was analyzed of peripheral blood indicators and myocardial remodeling indicators,including left atrial diameter(LAP),left ventricular posterior wall thickness(LVPW),left ventricle enddiastolic inner diameter(LVEDD),left ventricular mass index(LVMI),left ventricular remodeling index(LVRI),and LVEF correlation.Results The relative content of TFAM and lymphocyteβ1-AR mRNA in PBMCs was negatively correlated with LAP,LVPW,LVEDD,and LVMI,and positively with LVRI and LVEF.The relative content of GDF-15 mRNA was positively correlated with LAP,LVPW,LVEDD,and LVMI,and negatively with LVRI and LVEF(P<0.05).After follow-up for 3 months,the relative levels of TFAM and lymphocyteβ1-AR mRNA in PBMCs in the MACE group were found to be lower than those in the non-MACE group,and GDF-15 mRNA was higher in the MACE group than non-MACE group(P<0.05).The AUC value of TFAM,GDF-15,and lymphocyteβ1-AR mRNA combined was 0.889,which was greater than that of any index alone.Conclusion The abnormal expression of TFAM,GDF-15,and lymphocyteβ1-AR mRNA in PBMCs of CHF patients plays an important role in pathological changes.Their expression levels are closely related to MACE,and they may become new markers and therapeutic targets for CHF.
