支气管哮喘急性发作患儿FeNO、IgE水平与小气道功能的关系探讨 被引量:7

Relationship between FeNO,IgE levels and small airway function in children with acute attack of bronchial asthma

摘要 目的探讨支气管哮喘(哮喘)急性发作患儿呼出气一氧化氮(FeNO)、免疫球蛋白E(IgE)水平与小气道功能的关系。方法回顾性研究。选取2019年1~12月徐州医科大附属徐州儿童医院健康体检的50名小儿为对照组, 本院呼吸科同期收治的107例哮喘小儿为观察组。观察组根据临床症状及病史问询结果分为急性发作组(44例)和慢性持续组(63例)。对纳入研究小儿均行FeNO、IgE水平和小气道功能指标用力呼气25%肺活量的呼气流速(FEF25)、用力呼气50%肺活量的呼气流速(FEF50)、最大呼气肺活量的呼气流速(MMEF)检测。分析3组小儿的FeNO、IgE水平和小气道功能指标水平差异, 采用Pearson秩相关分析法分析FeNO、IgE水平和小气道功能指标的相关性。结果急性发作小儿FeNO水平与FEF25、FEF50、MMEF水平均呈负相关(r值分别为-0.906、-0.911、-0.884, P值均<0.05)。急性发作小儿IgE水平与FEF25、FEF50、MMEF水平均呈负相关(r值分别为-0.859、-0.856、-0.864, P值均<0.05)。3组小儿的FeNO、IgE水平均依次升高, 小气道功能指标(FEF25、FEF50、MMEF)均依次降低, 3组间两两比较, 差异均有统计学意义(P值均<0.05)。结论哮喘急性发作患儿FeNO、IgE水平与小气道功能具有相关性, 其可能在哮喘急性发作预测中具有一定的临床价值。 Objective To investigate the relationship between the levels of exhaled nitric oxide(FeNO),immunoglobulin E(IgE),and small airway function in children with acute bronchial asthma(asthma).Methods This was a retrospective study.Fifty children who underwent physical examination in Xuzhou Children′s Hospital affiliated to Xuzhou University of Medical Science from January to December 2019 were selected as the control group,and 107 children with asthma treated in respiratory department of our hospital during the same period were selected as the observation group.The observation group was divided into acute attack group(44 cases)and persistent attack group(63 cases)according to the results of clinical symptoms and medical history inquiry.FeNO and IgE levels and small airway function indexes of forced expiratory volume expiratory flow rate of 25%(FEF25),forced expiratory volume expiratory flow rate of 50%(FEF50)and maximum expiratory volume expiratory flow rate(MMEF)were detected.The differences of FeNO and IgE levels and small airway function indexes in the three groups were analyzed,and the correlation between FeNO and IgE levels and small airway function indexes was analyzed by Pearson rank correlation analysis.Results The level of FeNO was negatively correlated with FEF25,FEF50 and MMEF in children with acute attack(The r values were respectively-0.906,-0.911,-0.884,all P<0.05).IgE level was also negatively correlated with FEF25,FEF50 and MMEF levels in children with acute attack(The r values were respectively-0.859,-0.856,-0.861,all P<0.05).The levels of FeNO and IgE in the three groups were increased in turn,while the indexes of small airway function(FEF25,FEF50,MMEF)were decreased in turn.Pairwise comparison among the 3 groups showed statistically significant differences(all P<0.05).Conclusions FeNO and IgE levels are correlated with small airway function in children with acute asthma attack,which may have certain clinical value in the prediction of acute asthma attack.
