摘要
目的观察皱肺定喘膏联合西医常规治疗慢性阻塞性肺疾病(COPD)稳定期肺肾两虚证的疗效及安全性。方法将65例COPD稳定期患者随机分为对照组31例及观察组34例,对照组给予常规西医治疗,观察组在西医常规治疗基础上加服皱肺定喘膏,每次20 g,每日1次,连续服用10周。两组均从服药开始随访1年,治疗前及随访1年时(治疗后)检测肺功能指标[包括第一秒用力呼气的容积占预计值的百分比(FEV1%Pre)、用力肺活量占预计值百分比(FVC%Pre)、第一秒用力呼气量与用力呼气量百分比(FEV1/FVC)],比较慢性阻塞性肺疾病评估测试(CAT)评分、六分钟步行距离(6MWD)、随访期间急性加重次数,并观察不良反应。结果与本组治疗前比较,观察组治疗后6MWD、FEV1%Pre、FEV1/FVC升高,CAT评分降低(P<0.05或P<0.01),而对照组治疗后各指标差异无统计学意义(P>0.05)。治疗后组间比较,观察组6MWD、FEV1%Pre、FVC%Pre、FEV1/FVC均明显高于对照组,CAT评分低于对照组(P<0.05)。两组患者在随访期间急性加重次数比较差异无统计学意义(P>0.05),两组均未见明显药物不良反应。结论皱肺定喘膏联合西医常规治疗COPD稳定期肺肾两虚证患者,较单纯西医常规治疗,可明显改善患者肺功能、提高运动耐力及生活质量,且安全性好。
Objective combined with conventional western medicine in the treatment of stable chronic obstructive pulmonary disease(COPD)with lung-kidney deficiency pattern.MethodsSixty-five patients with stable COPD were randomly divided into two groups;31 cases in the control group were treated with conventional western medicine,while 34 cases in the treatment group were additionally given Zhoufei Dingchuan Ointment,20 g each time,once daily for 10 weeks.Both groups were followed up for one year since the initiation of medication,and the outcomes including the percentage of forced expiratory volume in the first second to the predicted value(FEV1%Pre),the percentage of forced vital capacity to the predicted value(FVC%Pre),the ratio of forced expiratory volume in the first second and the forced expiratory volume(FEV1/FVC)were assessed before treatment and after one year follow-up.The COPD assessment test(CAT)score,6-minute walking distance(6 MWD)and the frequency of acute exacerbation during follow-up were compared between groups,and the adverse reactions were recorded.ResultsCompared to those before treatment,6 MWD,FEV1%Pre and FEV1/FVC were significantly increased,and CAT score was reduced in the treatment group(P<0.05 or P<0.01),while those outcomes were not significantly changed in the control group(P>0.05).After treatment,the levels of 6 MWD,FEV1%pre,FVC%pre and FEV1/FVC in the treatment group were significantly higher than those in the control group(P<0.05),while the CAT score was significantly lower(P<0.05).There was no significant difference in the frequency of acute exacerbation between groups(P>0.05),and no significant drug-related adverse reactions were observed in both groups.ConclusionCompared to conventional western medicine alone,add-on Zhoufei Dingchuan Ointment can improve the lung function,enhance exercise endurance and improve the quality of life in stable COPD patients with lung-kidney pattern,and is safe.
