微创旋切活检术与空芯针穿刺活检术在乳腺疾病中的诊断价值对比 被引量:2

摘要 目的对比微创旋切活检术与空芯针穿刺活检术在乳腺疾病中的诊断价值。方法选择2017年2月—2019年1月本院收治的80例乳腺疾病患者作为研究对象,所有患者均行微创旋切活检术与空芯针穿刺活检术,以手术病理结果为金标准,对比微创旋切活检术与空芯针穿刺活检术在乳腺疾病中的诊断价值。结果经手术病理确诊,80例乳腺疾病患者中有165个结节,其中恶性结节40个,良性结节125个。良性乳腺肿瘤患者50例,其中8例为乳腺纤维腺癌,乳腺导管内乳头状瘤8例,乳腺不典型增生16例,乳腺腺病18例。乳腺癌患者30例,其中乳腺浸润性癌10例,导管内癌12例,乳腺小叶原位癌8例。微创旋切活检术诊断乳腺疾病的准确性为92.50%(74/80)、灵敏度为93.33%(28/30)、特异度为92.00%(46/50)、阳性预测值为87.50%(28/32)、阴性预测值为95.83%(46/48);空芯针穿刺活检术诊断乳腺疾病的分别为76.25%(61/80)、70.00%(21/30)、80.00%(40/50)、67.74%(21/31)、81.63%(40/49)。绘制ROC曲线,结果显示微创旋切活检术诊断乳腺疾病的AUC为0.927,>0.9为预测价值很好;空芯针穿刺活检术诊断乳腺疾病的AUC为0.750,>0.7为预测价值较好。结论微创旋切活检术诊断乳腺疾病的价值高于空芯针穿刺活检术诊断,可为临床治疗方案的制定提供依据。 Objective To compare the diagnostic value of minimally invasive rotary biopsy and hollow needle aspiration biopsy in diagnosis of breast diseases.Methods A total of 80 patients with breast diseases those admitted to our hospital in the period from February 2017 o January 2019 were taken as the research objects.All patients received minimally invasive rotary biopsy and hollow needle needle biopsy.The diagnostic value of minimally invasive rotary biopsy and hollow needle biopsy in breast diseases was compared with the pathological results of surgery as the gold standard.Results There were 165 nodules in 80 patients with breast diseases according to the pathological results of surgery,including 40 malignant nodules and 125 benign nodules.Among the 50 patients with benign breast tumor,there were 8 cases of fibroadenoma of breast,8 cases of intraductal papilloma,16 cases of atypical hyperplasia,and 18 cases of adenosis of breast.The accuracy of minimally invasive biopsy in diagnosis of breast disease was 92.50%(74/80),the sensitivity was 93.33%(28/30),and the specificity was 92.00%(46/50).The positive predictive value was 87.50%(28/32)and the negative predictive value was 95.83%(46/48).The rate of accuracy,sensitivity,specificity,positive predictive value,and negative predictive value in diagnosis of breast diseases by hollow needle aspiration biopsy was 76.25%(61/80),70.00%(21/30),80.00%(40/50),67.74%(21/31),and 81.63%(40/49),respectively.ROC curve was plotted,and the results showed that the AUC of minimally invasive rotary biopsy in the diagnosis of breast disease was 0.927,it is over 0.9 and means good prediction value.The AUC of hollow needle biopsy in the diagnosis of breast disease was 0.750,the prediction value is good.Conclusions The value of minimally invasive rotary biopsy in the diagnosis of breast diseases is better than that of hollow needle biopsy,which could provide a basis for the development of clinical treatment plan.
