引导式教育结合传统运动康复训练对脑性瘫痪患儿的影响 被引量:22

摘要 目的探讨引导式教育结合传统运动康复训练对脑性瘫痪患儿运动神经功能恢复和日常生活能力的影响。方法选取2019年5月至2020年5月于石家庄市人民医院康复科就诊并进行康复治疗的脑瘫患儿84例作为研究对象,随机分为对照组和观察组,各组42例。对照组进行传统运动康复训练,观察组采用引导式教育结合传统运动康复训练,比较两组患儿治疗3个月后的运动神经功能恢复情况,步行10m步数、时间及平衡能力,日常生活能力,综合功能评分情况。结果两组患儿治疗前的粗大运动功能量表(GMFM)评分、精细运动功能量表(FMFM)评分差异无统计学意义(P>0.05),治疗后观察组改善程度优于对照组(t值分别为2.126、2.044,P<0.05);治疗前两组患儿步行10m步数、时间及Berg平衡能力评分差异无统计学意义(P>0.05),治疗后观察组改善显著者优于对照组(t值分别为3.387、9.587、4.370,P<0.05);两组患儿治疗前的改良巴氏指数量表(ADL)评分差异无统计学意义(P>0.05),治疗后观察组患儿的ADL评分显著高于对照组(t=5.351,P<0.05);治疗前两组患儿的言语功能、认知功能、运动功能、自理动作、社会适应能力评分差异无统计学意义(P>0.05),治疗后观察组综合能力评分优于对照组(t值分别为4.272、2.924、2.791、4.005、3.030,P<0.05);观察组痉挛型与肌张力低下型的疗效均明显高于对照组,差异有统计学意义(χ^(2)值分别为5.927、5.769,P<0.05)。结论对脑性瘫痪患儿实施引导式教育结合传统运动康复训练,有助于患儿运动神经功能、平衡能力、日常生活能力的恢复,促进患儿心智的发育。 Objective To explore the effects of conductive education combined with traditional exercise rehabilitation training on the recovery of motor nerve function and ability of daily living in children with cerebral palsy.Methods 84 children with cerebral palsy who were treated and underwent rehabilitation in the department of rehabilitation of Shijiazhuang people′s hospital between May 2019 and May 2020 were selected as the research objects.They were randomly divided into a control group and an observation group,with 42 cases in each group.The control group was given traditional exercise rehabilitation training,and the observation group was given conductive education combined with traditional exercise rehabilitation training.The recovery of motor nerve function,step number,time and balance ability of walking 10 m,ability of daily living and comprehensive function scores of the two groups after three months of treatment were compared.Results There were no significant differences in the Gross Motor Function Measure(GMFM)score and Fine Motor Function Measure(FMFM)score between the two groups before treatment(P>0.05).After treatment,the improvements in the observation group were better than those in the control group(t=2.126 and 2.044,respectively,P<0.05).There were no significant differences in step number,time and Berg balance ability of walking 10 m between the two groups before treatment(P>0.05).After treatment,the improvements in observation group were significantly better than those in the control group(t=3.387,9.587 and 4.370,respectively,P<0.05).There was no significant difference in activities of daily living(ADL)score between the two groups before treatment(P>0.05),but the ADL score of observation group was significantly higher than that of the control group after treatment(t=5.351,P<0.05).There were no significant differences in speech function,cognitive function,motor function,self-care actions,and social adaptability scores between the two groups before treatment(P>0.05).After treatment,the comprehensive ab
