摘要
目的对2018—2020我院门诊电子处方进行点评,筛查出临床用药不合理的处方。方法对我院2018—2020年抽取的1 500张处方进行点评,分析电子处方的基本情况及其不合理用药情况。结果引进电子化处方点评系统后,我院医师处方合格率、基本药物品种占比明显上升,抗菌药物使用率、注射剂使用率明显下降,且上述指标均处于国家规定的标准范围内。1 500张门诊电子处方中,不合理用药处方共计208张(占13.87%),其中不规范电子处方、不适宜电子处方、超常电子处方分别为80张(占38.46%)、89张(占42.79%)、39张(占18.75%)。不规范电子处方中处方用量超7 d未注明延长时间、后记无签章发生率明显下降;不适宜电子处方中用法用量、给药频次不适宜以及给药途径不适宜发生率明显升高;超常电子处方中无适应证用药发生率明显升高。急诊科发生电子处方不合格率最高(36.06%),其次是妇科(14.42%)。结论实施电子处方点评后,处方合格率、处方点评质量均得到了明显持续的提高,具有显著的应用效果与价值。
Objective To comment on the e-prescriptions of our hospital’s outpatient department in 2018-2020, and screen out unreasonable clinical prescriptions. Methods Comment on 1 500 prescriptions selected in our hospital from 2018 to 2020, and analyze the basic situation of electronic prescriptions and their irrational use of drugs. Results After the introduction of the electronic prescription review system, the qualification rate of doctors’ prescriptions and the proportion of essential drugs in our hospital increased significantly, and the usage rate of antibacterial drugs and injections decreased significantly,and the above indicators were all within the scope of national standards. Among the 1 500 outpatient electronic prescriptions, there were 208 unreasonable drug prescriptions(13.87%). Among them, there are 80(38.46%), 89(42.79%) and 39(18.75%) of irregular electronic prescriptions, unsuitable electronic prescriptions, and abnormal electronic prescriptions. The incidence rate of "prescription dosage exceeding 7 days without indication of extension time" and "postscript without signature" in irregular electronic prescriptions has decreased significantly;unsuitable for electronic prescriptions "inappropriate usage,dosage and frequency of administration" and "inappropriate route of administration" the incidence has increased significantly;the incidence of "medication without indication" in supernormal electronic prescriptions has increased significantly. The emergency department has the highest rate of unqualified electronic prescriptions(36.06%), followed by gynecology(14.42%). Conclusion After the implementation of electronic prescription reviews, the prescription qualification rate and the quality of prescription reviews have been significantly and continuously improved, which has significant application effects and value.
