摘要
目的探讨益气固本、滋肾调免方剂对气阴两虚型免疫球蛋白A(IgA)肾病患者的疗效。方法采取随机数表法将郑州市第三人民医院2019年1月至2020年4月门诊及病房治疗的90例气阴两虚型IgA肾病患者分为对照组(45例)和研究组(45例)。对照组接受西医常规对症治疗,研究组在此基础上联合益气固本、滋肾调免方剂治疗。对比两组治疗前后中医症状评分、尿沉渣红细胞计数、24小时尿蛋白定量、血清尿素氮水平、血清肌酐水平、疗效、体液免疫学指标及不良反应发生率。结果治疗后,两组主症评分、次症评分、总分均下降,研究组评分较对照组低(P<0.05)。治疗后,两组尿沉渣红细胞计数、24小时尿蛋白定量、尿素氮、肌酐水平均降低,研究组各指标水平低于对照组(P<0.05)。治疗后,两组血清IgA水平降低,免疫球蛋白G(IgG)、补体3(C3)水平升高,研究组血清IgA水平低于对照组,IgG、C3水平高于对照组(P<0.05)。两组组内治疗前后、组间治疗前后IgM水平差异无统计学意义(P>0.05)。研究组治疗总有效率高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论益气固本、滋肾调免方剂可进一步提高气阴两虚型IgA肾病临床疗效,有效减轻患者症状,改善其肾功能,调节体液免疫功能。
Objective To explore the clinical effects of Yiqiguben Zishentiaomian prescription on immunoglobulin A(IgA)nephropathy of Qi and Yin deficiency type.Methods The random number table method was used to divide 90 patients with IgA nephropathy of Qi and Yin deficiency type in the outpatient and ward of Zhengzhou Third People’s Hospital from January 2019 to April 2020 into control group(45 cases)and study group(45 cases).Control group received conventional western medicine symptomatic treatment,and study group was treated with Yiqiguben Zishentiaomian prescription on this basis.The traditional Chinese medicine symptom score,urine sediment red blood cell count,24-hour urine protein quantification,serum urea nitrogen level,serum creatinine level,curative effect,humoral immunological indicators and incidence of adverse reactions were compared between the two groups.Results After treatment,the scores of primary symptoms,scores of secondary symptoms,and total scores of the two groups decreased,and the scores of study group were lower than those of control group(P<0.05).After treatment,the urine sediment red blood cell count,24-hour urine protein quantification,urea nitrogen level,and creatinine level decreased in the two groups and the above-mentioned index levels in study group were lower than those in control group(P<0.05).In the two groups,serum IgA levels decreased,and immunoglobulin G(IgG)and complement 3(C3)levels increased after treatment(P<0.05).Compared with control group,study group had lower serum IgA level and higher IgG and C3 levels(P<0.05).There was no statistical difference in IgM levels before and after treatment between the two groups(P>0.05).The total effective rate of treatment in study group was higher than that in control group(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yiqiguben Zishentiaomian prescription could further improve the clinical efficacy of IgA nephropathy of Qi and Yin deficiency type,effectively alleviate
