玻璃化冷冻技术积累卵母细胞在卵巢低反应患者中的应用分析 被引量:2

摘要 目的探讨玻璃化冷冻技术积累卵母细胞在卵巢低反应(POR)人群中的可行性及临床价值。方法回顾性分析2016年1月至2020年7月在我院因POR采用玻璃化冷冻技术积累卵母细胞,并于冷冻卵母细胞后下一周期新鲜取卵,将冷冻卵母细胞解冻与新鲜卵母细胞同时行卵胞浆内单精子注射(ICSI)的69例(138个周期)患者的临床资料。根据行ICSI的卵母细胞是否冻融分为冻卵组(n=121)和鲜卵组(n=190),比较两组患者的正常受精率、卵裂率、优质胚胎率,并分析冷冻卵母细胞解冻后的复苏情况;又根据移植胚胎来源不同分为3组:只移植冻卵来源胚胎的为冻卵胚胎组(9个周期),移植胚胎既有冻卵来源又有新鲜卵来源的为混合胚胎组(30个周期),只移植新鲜卵来源胚胎的为鲜卵胚胎组(27个周期),比较3组间的临床妊娠率。结果冻卵组共有冷冻MⅡ卵121枚,冷冻卵母细胞的复苏率为96.69%(117/121)。冻卵组和鲜卵组间正常受精率、卵裂率和优质胚胎率比较均无显著性差异(P>0.05);冻卵胚胎组、混合胚胎组及鲜卵胚胎组间平均年龄和平均移植胚胎数比较均无显著性差异(P>0.05);鲜卵胚胎组的临床妊娠率稍高于其他两组,但尚无显著性差异(P>0.05)。结论玻璃化冷冻技术可获得满意的卵母细胞复苏率及与新鲜卵相似的优质胚胎率,POR患者通过此技术可以累积卵母细胞,从而可获得更多可移植的优质胚胎。 Objective:To explore the clinical value of accumulation of oocytes with vitrification technology in patients with poor ovarian response(POR).Methods:The data of 69 POR patients(138 cycles)who received vitrification technology to accumulate oocytes in our hospital from January 2016 to July 2020 were retrospectively analyzed.The patients had their oocytes vitrified in the previous cycle,and the fresh oocytes were retrieved in next cycles.Both of the frozen-thawed and fresh oocytes were simultaneously performed with intracytoplasmic sperm injection(ICSI)at the same time.The cycles were divided into frozen-thawed oocytes group(n=121)and fresh oocytes group(n=190)according to whether oocytes were vitrified or not.The normal fertilization rate,cleavage rate and high-quality embryo rate were compared between the two groups.According to the transferred embryos derived from frozen-thawed oocytes or fresh oocytes,the cycles were divided into 3 groups:transferred embryos derived from frozen-thawed oocytes group(9 cycles),transferred embryos derived from both of frozen-thawed and fresh oocytes group(30 cycles),transferred embryos derived from fresh oocytes group(27 cycles),the clinical pregnancy rate were compared among the three groups.Results:A total of 121 MⅡoocytes were vitrified in the frozen-thawed oocytes group,the survival rate was 96.69%(117/121).There was no significant difference in normal fertilization rate,cleavage rate and high-quality embryo rate between frozen-thawed oocytes group and fresh oocytes group(P>0.05).There was no significant difference in average age and average number of embryos transferred among the embryos derived from frozen-thawed oocytes group,the embryos derived from mixed oocytes group and the embryos derived from fresh oocytes group(P>0.05).The clinical pregnancy rate of embryos derived from fresh oocytes group was slightly higher than that of the other two groups,but there was no significant difference(P>0.05).Conclusions:Vitrification technology can obtain content oocyte survival rate
