摘要
目的深入了解三级甲等医院造口治疗师与医师合作现状,分析医护合作的影响因素,并提出相应的干预策略。方法2019年7月至2020年6月,对涉及4个不同省市的三级甲等医院的13名造口治疗师进行面对面半结构式访谈,并采用Colaizzi 7步分析法对资料进行整理分析。结果针对造口治疗师与医师合作体验提炼出2个主题:不同身份的造口治疗师与医师的合作感受不同,即隶属于临床科室的造口治疗师能感受到更多合作与尊重,相对独立的专职造口治疗师与医师合作不多且不够积极;造口治疗师与医师合作关系受多方面因素影响,包括管理机制不完善、治疗理念存在冲突、传统医护关系的影响、工作时间交集少。结论医院方面应重视科室造口治疗师的培养与发展,完善会诊流程及队伍建设、健全绩效分配与医疗安全保障、提升职业认同感,全面促进造口治疗师与医师合作关系。
Objective To understand the status quo of cooperation between enterostomal therapists and physicians in Grade 3A hospitals,analyze the influencing factors of physician-nurse cooperation,and propose intervention countermeasures.Methods From July 2019 to June 2020,13 enterostomal therapists from Grade 3A hospitals in 4 provinces and cities were selected.They received a face-to-face semi-structured interview,and the data were collated and analyzed using Colaizzi's seven-step method.Results According to the experience of cooperation between enterostomal therapists and physicians,a total of 2 topics were extracted as follows:enterostomal therapists of different identities had different feelings of cooperation with physicians,that is,the enterostomal therapists in clinical departments felt a better attitude of cooperation and respect,while relatively independent full-time enterostomal therapists had less cooperation with physicians and were less proactive;the cooperation between enterostomal therapists and physicians was affected by many factors,including imperfect administrative mechanism,conflict in treatment concepts,influence by traditional physician-nurse relationship and less communication at work.Conclusion Hospitals should pay attention to the training and development of enterostomal therapists in their departments,improve the consultation process and team construction,build a well-established performance distribution and medical safety guarantee mechanism,enhance the sense of professional self-identity,and promote cooperation between enterostomal therapists and physicians.
