摘要
目的:采用超极速脉搏波(Ultrafast pulse wave velocity,UFPWV)技术定量评价女性桥本氏甲状腺炎(Hashimoto’s thyroiditis,HT)患者颈动脉弹性的变化,分析该变化与HT患者促甲状腺激素及甲状腺自身抗体水平的相关性,探讨HT病变的免疫反应与动脉硬化的关系。方法:选取73例甲状腺激素水平正常的女性HT患者,根据促甲状腺激素(TSH)水平分为甲功正常组(N1=47)、亚临床甲减组(N2=26),同时选取50例健康的女性体检者作为对照组,分别测定其甲状腺功能、甲状腺自身抗体水平、胆固醇(TC)及甘油三酯(TG)水平,使用UFPWV技术获取其颈动脉弹性参数,包括收缩期开始时的脉搏波传导速度(Beginning of systole,BS)和收缩期结束时的脉搏波传导速度(End of systole,ES)。结果:甲功正常组和亚临床甲减组BS与ES均高于对照组(P<0.01),且亚临床甲减组高于甲功正常组(P<0.05);甲功正常组和亚临床甲减组TgAb、TPOAb均高于对照组(P<0.05);亚临床甲减组TSH、TG、TC均高于甲功正常组及对照组(P<0.05),Pearson线性相关分析显示BS与TgAb、TPOAb、TSH呈正相关(r=0.518、0.551、0.357,P<0.05),ES与TgAb、TPOAb、TSH呈正相关(r=0.441、0.481、0.283,P<0.05)。结论:HT患者的免疫反应能加速动脉硬化的进程,UFPWV技术可作为评估女性HT患者颈动脉早期弹性变化的有效手段。
Objective:To quantitatively evaluate the changes of carotid artery elasticity in female patients with Hashimoto’s thyroiditis(HT)by ultrafast pulse wave velocity(UFPWV)technique,analyze the correlation between the changes and the level of thyroid stimulating hormone(TSH)and thyroid autoantibodies in HT patients,and to explore the relationship between the immune mechanism of HT pathogenesis and arteriosclerosis.Methods:A total of 73 female HT patients with normal thyroid hormone level were selected and divided into euthyroidism group(N1=47)and subclinical hypothyroidism group(N2=26),according to the level of TSH.And 50 healthy female subjects were selected as the control group.Their thyroid function,thyroid autoantibody,cholesterol(TC)and triglyceride(TG)levels were measured respectively.The carotid artery elasticity parameters by the UFPWV technique were obtained,including the pulse wave velocity at the beginning of systole(BS)and the end of systole(ES).Results:BS and ES of the euthyroidism group and the subclinical hypothyroidism group were higher than those of the control group(P<0.01),and those of the subclinical hypothyroidism group were higher than those of the euthyroidism group(P<0.05).TgAb,TPOAb of the euthyroidism group and the subclinical hypothyroidism group were higher than those of the control group(P<0.05);TSH,TG,TC of the subclinical hypothyroidism group was higher than those of the euthyroidism group and the control group(P<0.05).Pearson linear correlation analysis showed a positive correlation between BS and TgAb,TPOAb,TSH(r=0.518,0.551,0.357,P<0.05),ES was positively correlated with TgAb,TPOAb,TSH(r=0.441,0.481,0.283,P<0.05).Conclusion:The immune mechanism of HT can accelerate the process of atherosclerosis.UFPWV technique can be used as an effective method to evaluate the early changes of carotid artery elasticity in female HT patients.
