摘要
目的分析河北省康保县儿童腹泻细菌和病毒流行特征及病原学构成,为本地区感染性腹泻的防控提供科学依据。方法对康保县2014—2020年2家医院采集的5岁以内儿童感染性腹泻病例样本进行细菌和病毒检测,利用描述流行病学方法进行实验室检测结果的统计分析。结果2014—2020年共采集2260份腹泻病例标本,病原体总检出率为31.24%(706份)。细菌298份,其中致泻性大肠埃希菌48.66%(145/298),沙门菌27.85%(83/298),志贺菌16.11%(48/298),空肠弯曲菌7.38%(22/298);病毒408份,其中轮状病毒32.11%(131/408)、杯状病毒41.42%(169/408)、星状病毒2.21%(9/408)、腺病毒3.92%(16/408)、混合感染20.34%(83/408)。轮状病毒具有明显的季节高峰,其基因型主要为G3、G9、P8,星状病毒、腺病毒维持在较低水平。结论康保县儿童感染性腹泻病毒感染率高于细菌感染率,以A组轮状病毒和诺如Ⅱ型病毒为主要病毒,同时存在混合感染模式。
Objective To analyze the epidemiological characteristics and etiology of diarrhea in children in Kangbao County of Hebei Province,and to provide scientific basis for the prevention and control of infectious diarrhea in this area.Methods Bacteria and viruses were detected in samples of children with infectious diarrhea from two hospitals in Kangbao County from 2014 to 2020.Descriptive epidemiological method was used to analyze the results of laboratory tests.Results A total of 2260 diarrhea samples were collected from 2014 to 2020,and the total detection rate of pathogenic bacteria was 31.24%(706 cases).There were 298 cases of bacteria,including diarrhoeal Escherichia coli 48.66%(145/298),Salmonella 27.85%(83/298),Shigella 16.11%(48/298),Campylobacter jejuni 7.38%(22/298).There were 408 cases of virus,including rotavirus 32.11%(131/408),calicivirus 41.42%(169/408),astrovirus 2.21%(9/408),adenovirus 3.92%(16/408)and mixed infection 20.34%(83/408).Rotavirus has obvious seasonal peak,and its genotypes are mainly G3,G9 and P8.Astrovirus and adenovirus maintained a low level.Conclusion The infection rate of infectious diarrhea virus in children in Kangbao County was higher than that of bacteria.Group A rotavirus and norovirus type II were the main viruses,and there was a mixed infection mode at the same time.
