摘要
目的探讨白蛋白与球蛋白比值(AGR)对膀胱癌患者术后生存情况的预测作用。方法回顾性分析2010年3月至2013年5月郑州大学附属郑州中心医院收治的120例膀胱癌患者临床资料,计算AGR,通过受试者工作特征曲线(ROC)判断AGR最佳截断值并据此将患者分为高AGR组与低AGR组。采用Cox比例风险模型分析影响膀胱癌患者术后生存情况的危险因素。采用Kaplan-Meier法绘制生存曲线。结果120例患者中高AGR(≥1.58)组65例(54.17%),低AGR(<1.58)组55例(45.83%)。低AGR组患者年龄≥60岁占比(54.55%)高于高AGR组(33.85%)(P<0.05)。单因素分析显示,年龄≥60岁、肿瘤直径≥3 cm、多发肿瘤、高级别肿瘤、T3-T4期肿瘤及AGR值<1.58者生存时间短于年龄<60岁、肿瘤直径<3 cm、单发肿瘤、低级别肿瘤、T1-T2期肿瘤及AGR值≥1.58者(P<0.05)。Kaplan-Meier生存曲线显示,高AGR组与低AGR组生存曲线比较差异有统计学意义(P<0.05)。多因素Cox分析显示,肿瘤直径、肿瘤分级、肿瘤分期及AGR值是影响膀胱癌患者术后生存情况的危险因素(P<0.05)。结论AGR值是影响膀胱癌患者术后生存情况的独立危险因素,可作为膀胱癌患者预后的指标。
Objective To explore the role of albumin to globulin ratio(AGR)in predicting the survival of patients with bladder cancer.Methods The clinical data of 120 patients with bladder cancer were retrospectively analyzed.According to the receiver operating characteristic(ROC)curve,the patients were divided into the high AGR group and low AGR group.The postoperative survival of patients were analyzed with Cox proportional hazards model.The survival curve was drawn with Kaplan-Meier method.Results The proportion of patients aged≥60 years was higher in the low AGR group(54.55%)than in the high AGR group(33.85%)(P<0.05).Univariate analysis showed that patients with age≥60 years,tumor diameter≥3 cm,multiple tumors,high-grade tumors,T3-T4 tumors,and AGR<1.58 had a shorter survival time than those with age<60 years,tumor diameter<3 cm,single tumors,low-grade tumors,T1-T2 stage tumors,and AGR≥1.58(P<0.05).Kaplan-Meier survival curve showed that there was a statistically significant difference in survival between the two groups(P<0.05).Multivariate Cox analysis showed that tumor diameter,tumor grade,tumor stage and AGR were risk factors affecting the survival(P<0.05).Conclusion AGR is an independent risk factor affecting the survival of patients with bladder cancer,and can be used as an indicator of the prognosis.
