摘要
目的:提出一种基于感兴趣区(region of interest,ROI)的呼吸机潮气量准确性评价方法。方法:根据低潮气量通气原则及待检呼吸机潮气量输出范围,从2017—2019年某院ICU收治的3964名患者中筛选出3598名患者,获取其体质量数据频率分布。选取体质量数据累积百分比10%、30%、50%、70%及90%处的体质量,即2.5、3.9、6.0、10.1、19.0 kg,按8 mL/kg的标准及待检呼吸机、气流分析仪的精度信息计算并获取各处对应的潮气量为24、54、84、114、160 mL。以该潮气量作为潮气量校准点、以数据累积百分比10%及90%处体质量对应的潮气量区间为ROI对某院在用的10台呼吸机进行检测,并将检测获得的潮气量误差结果与按照JJF 1234—2018《呼吸机校准规范》校准方法获得的结果进行比较。结果:2种检测方法检出的呼吸机潮气量的误差范围均符合国家计量标准检测规范的要求,但该研究提出的方法可提供更多ROI内呼吸机潮气量的精度信息。结论:该方法依照患者体质量数据频率分布选取潮气量校准点,可实现呼吸机潮气量的差异化检测。
Objective To propose a region of interest(ROI)-based method for evaluating the accuracy of the ventilator tidal volumes.Methods Totally 3598 patients were screened from 3964 patients admitted to the ICU of some hospital from 2017—2019 to determine the frequency distribution of their body mass data based on the principles of low tidal volume ventilation and the tidal volume output range of the ventilator to be tested.The body masses corresponding to 10%,30%,50%,70%and 90%cumulative percentage of body mass data,i.e.2.5,3.9,6.0,10.1 and 19.0 kg,were selected,and the corresponding tidal volumes of 24,54,84,114 and 160 mL were calculated and obtained according to the standard of 8 mL/kg and the accuracy information of the ventilator and airflow analyzer to be tested.Ten ventilators in use in some hospital were tested with the tidal volumes obtained as the calibration points and the tidal volume intervals between the body masses corresponding to 10%and 90%cumulative percentage as the ROI,and the tidal volume error results got from the tests were compared with those acquired according to JJF 1234—2018"Calibration Specification for Ventilators".Results The error ranges of the ventilator tidal volumes detected by the 2 testing methods met the requirements of the national metrological standard testing specifications,while the method proposed provided more information on the accuracy of the ventilator tidal volume within the ROI.Conclusion The method proposed selects tidal volume calibration points based on the frequency distribution of the patient's body mass data,enabling differential detection of ventilator tidal volumes.
