摘要
目的探究生酮饮食添加对难治性癫痫患儿血管内皮功能及T淋巴细胞亚群的影响。方法前瞻性选取2018年9月至2020年9月海南省妇女儿童医学中心收治的难治性癫痫患儿68例,按随机数字表方法分为对照组和观察组,每组各34例。对照组患儿进行常规抗癫痫治疗,观察组患儿在对照组的基础上采用生酮饮食。检测治疗前、治疗3个月后的血脂水平[甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)],计算动脉粥样硬化风险指数;检测治疗前、治疗3个月后血管内皮功能[一氧化氮(NO)、内皮素-1(ET-1)]及T淋巴细胞水平(包括CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))。结果治疗3个月后,观察组血清中LDL水平为(3.41±1.33)mmol/L,明显高于对照组[(2.78±1.11)mmol/L],差异有统计学意义(P<0.05),TC、HDL、LDL水平较对照组差异无统计学意义(P>0.05)。治疗3个月后,观察组LDL/HDL、AI、LCI数值均有升高,AIP降低,但仅有观察组LDL/HDL(3.12±1.08)与对照组(2.21±0.67)相比,差异有统计学意义(P<0.05)。治疗3个月后,2组NO、ET-1水平比较,差异均无统计学意义(P>0.05)。治疗3个月后,观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平分别为(0.78±0.25)%、(0.55±0.18)%、1.25±0.29,高于对照组[(0.46±0.15)%、(0.28±0.14)%、0.86±0.20],观察组CD8^(+)水平为(0.46±0.08)%,低于对照组[(0.56±0.16)%],差异均有统计学意义(P<0.05)。结论生酮饮食添加可升高难治性癫痫患儿LDL水平,使LDL/HDL比值升高,但未对血管内皮功能造成明显损伤,明显改善患儿T细胞亚群水平。
Objective To investigate the effect of ketogenic diet on vascular endothelial function and T lymphocyte subsets in children with refractory epilepsy.Methods A total of 68 children with intractable epilepsy admitted to the Hainan Women's and Children's Medical Center from September 2018 to September 2020 were prospectively selected and randomly divided into control group and observation group with 34 cases in each group.The children in the control group was received routine antiepileptic treatment,while the children in the observation group was received ketogenic diet on the basis of the control group.Blood lipid levels including triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL)were detected before and after treatment,atherosclerosis risk index was calculated,vascular endothelial function nitric oxide(NO),endothelin-1(ET-1)and T lymphocyte levels(including CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))were detected before and after 3 months treatment.Results After 3 months of treatment,the level of serum LDL in the observation group was(3.41±1.33)mmol/L,which was significantly higher than that in the control group[(2.78±1.11)mmol/L],the difference was statistically significant(P<0.05),there was no statistically significant difference in the levels of TC,HDL,LDL compared with the control group(P>0.05).After 3 months of treatment,compared with the control group,LDL/HDL,AI and LCI values in the observation group increased and AIP decreased,but there was no statistically significant difference(P>0.05),the level of LDL/HDL in the observation group(3.12±1.08)was higher than that in the control group(2.21±0.67),and the difference was statistically significant(P<0.05).After treatment,there was no significant difference in NO and ET-1 levels between the two groups(P>0.05).After 3 months of treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the observation group were(0.78±0.25)%,(0.55±0.18)%,1.25±0.29,respectively,which were higher than the control group[(0.46±0.15)%,
