早期补充甘油磷酸钠对极低出生体重早产儿骨代谢及体格发育的影响 被引量:3

摘要 目的探讨早期补充甘油磷酸钠对极低出生体重早产儿骨代谢及体格发育的影响,为临床补充矿物质预防早产儿代谢性骨病(MBD)提供依据。方法回顾东莞市人民医院儿科2019年1月至2020年6月住院治疗的极低出生体重早产儿128例,其中早期补充甘油磷酸钠治疗的为补磷组65例,无补充甘油磷酸钠的为对照组63例。比较两组出生后48 h内、出生后1周、2周、3周、4周、6周及出院时血清钙(Ca^(2+))、血清磷(Pi)、碱性磷酸酶(ALP)水平,以及出院时两组的体格发育指标。结果两组在性别比例、平均出生胎龄、体重、身长、头围上差异无统计学意义(P>0.05)。两组在出生后48 h和出生后1周时的血清Ca^(2+)、Pi、ALP水平比较差异无统计学意义(P>0.05);出生后2周时补磷组的血清Ca^(2+)、Pi水平高于对照组(P<0.05);出生后3周、4周、6周及出院时补磷组的Pi水平明显高于对照组,而ALP水平则明显低于对照组(P<0.05)。出院时补磷组的身长和头围水平高于对照组(P<0.05)。结论出生后早期补充甘油磷酸钠使极低出生体重早产儿的Pi水平更高,ALP水平更低,出院时的身长和头围水平更高。出生后早期补充磷元素可改善极低出生体重早产儿的骨代谢状态,同时促进早产儿的骨骼生长。 Objective To investigate the effects of early supplementation of sodium glycerophosphate on bone metabolism and physical development in very low birth weight preterm infants,and to provide a basis for clinical mineral supplementation for the prevention of metabolic bone disease(MBD)in preterm infants.Methods A total of 128 cases preterm infants of very low birth weight hospitalized in the neonatology department of Dongguan People's Hospital from January 2019 to June 2020 were reviewed,of which 65 cases were treated with early supplementation of sodium glycerophosphate as the phosphorus supplementation group and 63 cases were treated without supplementation of sodium glycerophosphate as the control group.Serum calcium(Ca^(2+)),serum phosphorus(Pi)and alkaline phosphatase(ALP)levels within 48 hours after birth,1 week,2 weeks,3 weeks,4 weeks,6 weeks after birth and at the time of discharge were compared between the two groups,and physical developmental indices of the two groups at the time of discharge were also compared.Results There were no statistically significant differences between the two groups in sex ratio,mean gestational age at birth,weight,length,and head circumference(P>0.05).There were no statistically significant differences in the serum Ca^(2+),Pi and ALP levels between the two groups at 48 hours and 1 week after birth(P>0.05).Serum Ca and P levels were higher in the phosphorus supplementation group than those in the control group at 2 weeks after birth(P<0.05).Serum P levels were significantly higher in the phosphorus supplementation group than those in the control group at 3,4 and 6 weeks after birth and at the time of discharge,while ALP levels were significantly lower than those in the control group(P<0.05).The length and head circumference levels were higher in the phosphorus-supplemented group than those in the control group at discharge(P<0.05).Conclusion Early postnatal supplementation with sodium glycerophosphate resulted in higher serum Pi levels,lower serum ALP levels,and higher length and
