摘要
目的观察消渴痹足方足浴联合西医治疗老年气虚血瘀型糖尿病周围神经病变(DPN)的临床疗效。方法将80例气虚血瘀型老年DPN患者按照随机数字表法分为2组,对照组40例予西医常规治疗,治疗组40例在对照组基础上联合消渴痹足方足浴治疗。2组均治疗4周后统计疗效,比较2组治疗后多伦多临床评分系统(TCSS)评分变化情况,包括神经症状、神经反射及感觉功能,比较2组治疗前后正中神经和腓总神经的运动神经传导速度(MCV)和感觉神经传导速度(SCV)变化情况。结果治疗组总有效率95.0%(38/40),对照组77.5%(31/40),治疗组疗效优于对照组(P<0.05)。2组治疗后TCSS神经症状、神经反射及感觉功能评分与本组治疗前比较均明显降低(P<0.05),且治疗组治疗后各项评分均低于对照组(P<0.05)。2组治疗后正中神经和腓总神经MCV、SCV与本组治疗前比较均明显升高(P<0.05),且治疗组治疗后各指标均高于对照组(P<0.05)。结论消渴痹足方足浴联合西医治疗老年气虚血瘀型DPN临床疗效确切,可明显改善患者神经受损症状、体征,促进病情恢复,提高正中神经和腓总神经传导速度。
Objective To observe the clinical efficacy of foot bath with XiaokeBizu Formula combined with western medicine for elderly patients with diabetic peripheral neuropathy(DPN)of qi deficiency and blood stasis.Methods Totally 80 elderly patients with DPN of qi deficiency and blood stasis were randomly divided into two groups,40 patients in control group received conventional western medicine,and 40 patients in treatment group additionally received therapy of foot bath with XiaokeBizu Formula on the basis of control group.Treated 4 weeks,the key observation was toronto clinical scoring system(TCSS)score(nervous symptoms,nervous reflex and sensory function),and the change of motor nerve conduction velocity(MCV)and sensory conduction velocity(SCV)of median and common peroneal nerve.Results The total effective rates in treatment group were better than those in control group[95.0%(38/40)vs 77.5%(31/40),(P<0.05),respectively].Compared with before treatment,TCSS scores(nervous symptoms,nervous reflex and sensory function)in groups were lower after treatment(P<0.05),which in treatment group were significantly decreased(P<0.05);MCV and SCV of median nerve and common peroneal nerve in groups were higher after treatment(P<0.05),which in treatment group were significantly increased(P<0.05).Conclusion For elderly patients with DPN of qi deficiency and blood stasis,XiaokeBizu Formula combined with western medicine has a definite curative effect,which can significantly improve symptoms of nerve damage and signs,promote recovery of disease,increase conduction velocity of median and common peroneal nerve,and improve curative effect.
