摘要
目的比较腰硬联合麻醉及连续硬膜外麻醉应用于顺产经产妇分娩镇痛的镇痛效果。方法回顾性分析2019年1-12月收治的118例顺产经产妇分娩镇痛病例的临床资料,根据实施麻醉方法的不同分为腰硬联合麻醉组(A组,n=56)和连续硬膜外麻醉组(B组,n=62),对两组产妇的镇痛药物用量、分娩镇痛效果、产程、不良反应发生情况、剖宫产率、产钳助产率及新生儿出生后1 min Apgar评分进行比较。结果与B组比较,A组麻醉药用量减少,药物起效时间、第一及第二产程时间明显缩短,不良反应的发生率及产妇剖宫产率、产钳助产率降低,差别具有统计学意义(P<0.05)。结论腰硬联合麻醉应用于顺产经产妇无痛分娩具有麻醉用药剂量减少、起效快、不良反应少、剖宫产率及产钳助产率降低等优点,具备推广价值。
Objective To analyze the analgesic effects of combined spinal-epidural anesthesia and continuous epidural anesthesia on labor pain of parturient.Methods Retrospective analysis was made on the clinical data of 118 cases of analgesia during childbirth in the hospital during January to December 2019.According to the different methods of anesthesia,the patients were divided into two groups:Combined spinal-epidural anesthesia group(group A,n=56)and continuous epidural anesthesia group(group B,n=56).The dose of analgesic drugs and analgesic effect of labor,the progress of labor and the occurrence of adverse reactions,the rate of cesarean section and forceps assisted delivery,the Apgar scores of neonatal at 1 min after birth in the two groups were statistically analyzed.Results Compared with group B,the dosage of anesthetics in group A was reduced,the time of drug onset and the time of the first and second stages of labor were significantly shortened.The incidence of adverse reactions and the rate of cesarean section and forceps assisted delivery in group A were lower than that in group B.The differences were statistically significant(P<0.05).Conclusion Combined spinal-epidural anesthesia can reduce the dosage of anesthetic drugs,shorten the labor process,reduce the occurrence of adverse reactions and the rate of cesarean section and forceps assisted delivery.It is worthy of clinical application and promotion for labor analgesia of normal parturient.
