摘要
目的探讨杜非合剂联合利多卡因局部麻醉在输尿管软镜碎石取石术的可行性和安全性。方法选取大余县人民医院20例输尿管上段、肾结石患者,结石≤2 cm;在杜非合剂联合利多卡因局部麻醉下行输尿管软镜碎石取石术,分析患者术中、术后疼痛耐受情况及疗效。结果20例患者均Ⅰ期手术成功,术中未出现因无法耐受麻醉而中转全麻插管情况,术中、术后无输尿管损伤穿孔、出血、感染等并发症;T1时患者HR、SBP、DBP均较T0时降低,T2、T3时稍高于T0时,但各时点对比差异均无统计学意义(P>0.05)。结论输尿管上段、肾结石患者于杜非合剂联合利多卡因局部麻醉下行输尿管软镜碎石取石术,在满足手术麻醉要求的同时,对患者全身影响小,是一种安全有效的麻醉方式,值得临床推广。
Objective To investigate the feasibility and safety of Dufei Mixture combined with lidocaine local anesthesia in flexible ureteroscopic lithotripsy.Methods Twenty patients with upper ureteral and renal calculi,with calculi less than 2 cm,were treated with flexible ureteroscopic lithotripsy under local anesthesia with Dufei Mixture combined with lidocaine.The intraoperative and postoperative pain tolerance and efficacy were analyzed.Results All patients(n=20)finished the one-stage surgery successfully.None was transferred to general anesthesia due to intolerance of anesthesia,or had complications like perforation,hemorrhage or infection of ureter injury during and after operation.HR,SBP and DBP at T1 were lower than those at T0,and slightly higher than those at T0 at T2 and T3,but there was no significant difference between the two groups(P>0.05).Conclusion Flexible ureteroscopic lithotripsy under local anesthesia with Dufei Mixture combined with lidocaine in patients with upper ureter and kidney stones is a safe and effective anesthesia method with little effect on the patient's general body while meeting the requirements of surgical anesthesia,which is worthy of clinical promotion.
