摘要
目的:观察温经活血汤加减联合壮医药线点灸治疗子宫腺肌病(AM)慢性疼痛的临床疗效。方法:选取50例AM慢性疼痛患者,给予温经活血汤加减联合壮医药线点灸治疗,3个月经周期为1个疗程,共治疗2个疗程。比较患者治疗前后痛经视觉模拟评分法(VAS)评分、COX痛经症状评分量表(CMSS)评分、排卵痛VAS评分、排卵痛持续时间、慢性盆腔痛评分、月经量、子宫内膜厚度、子宫体积、血清糖类抗原(CA) 125水平及生活质量。结果:治疗1个疗程、2个疗程后,患者痛经VAS评分、CMSS痛经程度评分、CMSS痛经时间评分、排卵痛VAS评分、慢性盆腔痛评分均较治疗前降低(P<0.05),排卵痛持续时间均较治疗前缩短(P<0.05)。治疗1个疗程、2个疗程后,患者月经失血图评分法(PBAC)评分、血清CA125水平均较治疗前降低(P<0.05),子宫内膜厚度较治疗前减小(P<0.05);患者子宫体积与治疗前比较,差异无统计学意义(P>0.05);患者下腹疼痛、腰骶酸胀、肛门坠胀、恶心呕吐、头晕乏力、睡眠均较治疗前改善(P<0.05);患者焦虑、社交及心理压力承受与治疗前比较,差异均无统计学意义(P>0.05)。结论:温经活血汤加减联合壮医药线点灸治疗AM慢性疼痛疗效肯定,可明显缓解痛经症状,缩短痛经及排卵痛持续时间,减少月经量,改善生活质量。
Objective:To observe the clinical effect of modified Wenjing Huoxue tang combined with medicated thread moxibustion of Zhuang medicine on the chronic pain in adenomyosis(AM). Methods:A total of 50 patients with AM chronic pain were selected and treated with modified Wenjing Huoxue tang combined with medicated thread moxibustion of Zhuang medicine. Three menstrual cycles were one course of treatment,and a total of two courses of treatment were given. The Visual Analogue Scale(VAS) score,Cox Menstrual Symptom Scale(CMSS) score,VAS score of ovulation pain,duration of ovulation pain, score of chronic pelvic pain, menstrual volume, endometrial thickness, uterine volume, carbohydrate antigen(CA) 125 in serum and quality of life were compared before and after treatment. Results: After one course of treatment and two courses of treatment,the VAS score,score of dysmenorrhea degree in CMSS,score of dysmenorrhea time in CMSS,score of ovulation pain in VAS and score of chronic pelvic pain were all decreased when compared with those before treatment(P<0.05);the duration of ovulation pain was shorter than that before treatment(P<0.05). After one course of treatment and two courses of treatment,the score of Pictorial Blood Loss Assessment Chart(PBAC) of patients and the level of CA125 in serum were decreased when compared with those before treatment(P<0.05);the endometrial thickness was decreased when compared with that before treatment(P<0.05);there was no significant difference in uterine volume between the patients and before treatment(P>0.05);the lower abdominal pain,lumbosacral acid distension,anal swelling,nausea and vomiting, dizziness and fatigue, and sleep were improved when compared with those before treatment(P<0.05);there was no significant difference in anxiety,social and psychological stress of patients when compared with those before treatment(P>0.05).
