摘要
目的:通过数据挖掘探讨针灸治疗癌性疼痛的取穴规律。方法:检索自建库到2020年5月30日期间中国知网、万方、维普平台中有关针灸治疗癌性疼痛的中文文献,将符合标准的文献录入EXCEL数据库中,并用SPSS20.0统计软件进行数据分析。结果:共检索到71篇文献,在未区分具体癌症进行治疗的针灸处方中,胃经是最常选用的经脉,其次为大肠经、脾经。使用频次由多到少依次排序的5个腧穴分别为足三里、合谷、阿是穴、三阴交和内关,使用特定穴以五输穴、合穴、下合穴为主。针对不同脏腑癌痛进行治疗的针灸处方中,胃癌癌痛常用腧穴为足三里、中脘;肺癌癌痛常用腧穴为内关、肺俞、足三里;肝癌癌痛常用腧穴为太冲、期门、肝俞、三阴交。所选经脉以病变脏腑所属经脉和膀胱经为主,特定穴使用则以背俞穴、募穴、合穴为主。将使用频次≥5的高频穴位进行聚类分析,最终得出4类结果。结论:针灸治疗癌痛,以合谷、足三里、内关、三阴交、阿是穴为基本穴位处方。根据不同脏腑的癌痛情况,应以病变脏腑所属经脉之合穴、募穴配合膀胱经相应的背俞穴为基础,联合应用基本穴位处方随症加减治疗。
Objective:To discuss the point selection rules of acupuncture and moxibustion for cancer pain by data mining.Methods:The Chinese literature on acupuncture and moxibustion for cancer pain in CNKI,Wanfang and VIP was retrieved from the establishment of the databases to May 30,2020.The documents in line with the standards were entered into Excel database and the data was analyzed with SPSS20.0 statistical software.Results:A total of 71 literature was retrieved.Among the acupuncture prescriptions without distinguishing specific cancers,stomach channel is the most commonly used channel,followed by large intestine channel and spleen channel.The five acupuncture points in order of their frequency from more to less are Zusanli(ST 36),Hegu(LI 4),Ashi,Sanyinjiao(SP 6)and Neiguan(PC 6)points,and the specific points are mainly five transport points,he-sea point and lower he-sea points.In the acupuncture prescriptions for cancer pain in different viscera,the commonly used acupuncture points for gastric cancer pain are Zusanli(ST 36)and Zhongwan(CV 12)points;the common points for lung cancer pain are Neiguan(PC 6),Feishu(BL 13)and Zusanli(ST 36)points;the common points for liver cancer pain are Taichong(LR 3),Qimen(LR 14),Ganshu(BL 18)and Sanyinjiao(SP 6)points.The selected channels are mainly the channels of the diseased viscera and the bladder channel,while the specific points are mainly backshu point,front-mu point and he-sea point.The cluster analysis was carried out with high frequency points whose frequency is greater than or equal to five,and finally four kinds of results were obtained.Conclusion:Acupuncture and moxibustion treats cancer pain,with Zusanli(ST 36),Hegu(LI 4),Ashi,Sanyinjiao(SP 6)and Neiguan(PC 6)points being the basic point prescriptions.According to the cancer pain of different viscera,the basic point prescriptions modified with symptoms should be combined based on the combination of the he-sea point and front-mu point in the channel of the diseased viscera and the corresponding back-shu point of bladder
