摘要
目的探讨鼻内镜下行高选择性翼管神经分支等离子射频消融术治疗过敏性鼻炎的临床疗效.方法将2019年1月—2020年5月浦北县人民医院收治的98例过敏性鼻炎患者按随机数字表法分为2组,对照组48例采用传统翼管神经切断术治疗,观察组50例采用鼻内镜下行高选择性翼管神经分支低温等离子射频消融术治疗,比较2组患者临床疗效、手术指标、并发症发生情况,以及术后1周、3个月、6个月时患者VAS评分和RQLQ评分.结果2组总有效率比较差异无统计学意义(P>0.05),但观察组显效率明显高于对照组(P<0.05).观察组手术时间、术后住院时间、术中失血量均明显少于对照组(P<0.05).与术前相比较,2组患者术后1周、术后3个月、术后6个月VAS评分、RQLQ评分均有明显改善且观察组各时点VAS评分、RQLQ评分均明显低于对照组(P<0.05).观察组术后总并发症发生率明显低于对照组(P<0.05).结论鼻内镜下行高选择性翼管神经分支等离子射频消融术治疗过敏性鼻炎能获得良好疗效,具有失血量少、痛苦轻、住院时间短及术后并发症发生率低等优点,值得临床推广.
Objective To investigate the clinical effect of highly selective branches neurotomy of vidian verve with plasma radiofrequency ablation under endoscopy in treatment of allergic rhinitis.Methods 98 cases of allergiv rhinitis in our hospital from January 2019 to May 2020 were randomly divided into two group,48 cases in the control group were treated with traditional wing tube neurectomy,and 50 cases in the control group to take the highly selective branches neurotomy of vidian verve with plasma radiofrequency ablation under endoscopy.The clinal effect,operation index,complication and the VAS scores,RQLQ scores in one week,three months and six months after operation were compared between the two groups.Results There was no significantly difference between the twogroups(P>0.05),but the excellence rate in the observation group was significantly higher than that in the control group(P<0.05).The operation time,the postoperative hospital stay and intraoperative blood loss in the observation group was significantly shorter or lower than that in the control group(P<0.05).The VAS scores and RQLQ scores of the two groups during one week,three months and six months after operation were significantly improve than those in preoperative(P<0.05).The VAS scores and RQLQ scores of the obsevation group during one week,three months and six months after operation were significantly lower than those in the control group(P<0.05).The total incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Highly selective branches neurotomy of vidian verve with plasma radiofrequency ablation under endoscopy can achieve good curative effect in treatment of allergic rhinitis.It has advantages of less blood loss,less pain,shorthospitalstay and low incidence of postoperative complications,and is worth to be applied for clinical practive.
