摘要
目的观察造口术前定位对改善肠造口患者生活质量的效果。方法便利选取该院2019年1月—2020年6月收治的86例行肠造口手术的患者为该次研究对象,在患者自愿参与该次研究的前提下通过双盲随机法分为对照组(43例:未行术前定位)与研究组(43例:行造口术前定位),比较两组患者生活质量,并发症发生率及治疗效果。结果研究组中Frankl依从性级别为4分患者所占比例为62.79%,明显高于对照组37.21%,差异有统计学意义(Z=2.372,P<0.05)。研究组刺激性皮炎1例(2.33%)、造口旁疝1例(2.33%)、造口脱垂0例(0.00%),术后并发症发生率为4.66%,相较于对照组刺激性皮炎4例(9.30%)、造口旁疝4例(9.30%)、造口脱垂2例(4.66%),术后并发症发生率23.26%明显要低,差异有统计学意义(χ^(2)=6.198,P<0.05)。研究组术后3个月生活质量受到重度影响者所占比3例(6.97%)明显低于对照组12例(27.91%),差异有统计学意义(Z=-2.558,P<0.05)。结论行造口术前定位的患者围手术期治疗依从性高,术后并发症发生率低,且生活质量明显优于未行术前定位的患者,有较高的临床推广价值。
Objective To observe the effect of preoperative positioning of ostomy on improving the quality of life of patients with enterostomy.Methods Eighty-six patients undergoing enterostomy surgery admitted to the hospital from January 2019 to June 2020 were conveniently selected as the study subjects.On the premise of voluntary participation in this study,the patients were divided into the control group by double-blind randomization(43 cases,no preoperative positioning)and the study group(43 cases,preoperative positioning)to compare the quality of life,complication rate and treatment effect of the two groups.Results In the study group,the proportion of patients with Frankl compliance level of 4 points was 62.79%,which was significantly higher than 37.21%in the control group.The difference was statistically significant(Z=2.372,P<0.05).the study group had 1 case of irritant dermatitis(2.33%),1 case of parastoma hernia(2.33%),and 0 case of stoma prolapse(0.00%).The postoperative complication rate was 4.66%.Compared with the control group,the complication rate was 4.66%.4 cases of dermatitis(9.30%),4 cases of parastoma hernia(9.30%),and 2 cases of stoma prolapse(4.66%).The postoperative complication rate of 23.26%was significantly lower,and the difference was statistically significant(χ^(2)=6.198,P<0.05).The proportion of patients whose quality of life was severely affected 3 months after the operation in the study group 3 cases(6.97%)was significantly lower than that of the control group 12 cases(27.91%),and the difference was statistically significant(Z=-2.558,P<0.05).Conclusion Patients with preoperative positioning have high perioperative treatment compliance,low postoperative complications,and the quality of life is significantly better than that of patients without preoperative positioning,which has high clinical promotion value.
