摘要
目的:探讨医护患责任联动护理干预方案在肠造口患者院外康复管理中的应用效果。方法:采用便利抽样法,选取2016年1月—2020年8月青海省人民医院收治的永久性结直肠癌造口患者为研究对象,根据入院先后顺序分为对照组和干预组,每组76例。对照组采用常规护理,干预组实施医护患责任联动护理干预方案。比较两组患者干预后造口自我护理量表得分、造口并发症发生率,并比较两组患者干预前后中文版造口患者生活质量量表(C-COH)得分。结果:干预组患者干预后造口自我护理量表条目均分为(4.25±0.61)分,C-COH条目均分为(7.60±1.26)分,高于对照组的(3.69±0.67)、(6.95±1.35)分,差异有统计学意义(t值分别为5.387、3.068;P<0.05)。干预组患者干预后造口并发症发生率为7.8%(6/76),低于对照组的19.7%(15/76),差异有统计学意义(χ^(2)=4.475,P<0.05)。结论:将医护患责任联动应用于肠造口患者院外康复管理中,能提高患者的自我护理能力,降低并发症发生率,改善生活质量,值得推广。
Objective To explore the effect of the doctors,nurses and patients responsibility linkage nursing plan in the out-of-hospital rehabilitation management of patients with enterostomy.Methods From January 2016 to August 2020,convenience sampling was used to select permanent colorectal cancer stoma patients admitted to Qinghai Provincial People's Hospital as research objects.According to the order of admission,patients were divided into the control group and the intervention group,with 76 cases in each group.The control group carried out routine nursing,and the intervention group implemented the doctor,nurse and patient responsibility linkage nursing plan.The scores of the Stoma Self-care Scale and the incidence of stoma complications were compared between the two groups of patients after the intervention,and the scores of the Chinese Version of City of Hope Quality of Life-Ostomy Questionnaire(C-COH)of patients with stoma before and after the intervention were compared between the two groups.Results After the intervention,patients in the intervention group had an item average score of(4.25±0.61)on the Stoma Self-care Scale,and an item average score of(7.60±1.26)on C-COH,which were higher than those[(3.69±0.67)and(6.95±1.35)]in the control group,the differences were statistically significant(t=5.387,3.068;P<0.05).The incidence of stoma complications in the intervention group after the intervention was 7.8%(6/76),which was lower than 19.7%(15/76)in the control group,and the difference was statistically significant(χ^(2)=4.475,P<0.05).Conclusions The application of the doctors,nurses and patients responsibility linkage nursing plan in the out-of-hospital rehabilitation management of patients with enterostomy can improve the self-care ability of patients,reduce the incidence of complications,and improve the quality of life,which is worthy of promotion.
