摘要
基于癌毒病机理论,笔者团队提出结直肠腺瘤分期辨治的观点。参照世界卫生组织对结直肠腺瘤的病理分期,将结直肠腺瘤进行临床分期,辨析不同分期的核心病机,提出相应的治则治法。脾气亏虚,湿痰瘀互结是早期结直肠腺瘤的核心病机,治当健脾利湿、化痰祛瘀散结,重在消瘤散结,防治腺瘤复发增生;脾气亏虚,湿痰瘀毒互结是晚期结直肠腺瘤癌变的核心病机,治当健脾利湿、化痰祛瘀、抗癌解毒,重在抗癌解毒,防治腺瘤癌变。主要治法包括:健脾益气、化湿泄浊、化痰散结、祛瘀消癥、抗癌解毒。
Based on the theory of cancer toxin pathogenesis,the team proposed the view of staging and differentiating colorectal adenoma.According to WHO’s pathological staging of colorectal adenoma,clinical staging of colorectal adenoma is carried out,the core pathogenesis of different stages is differentiated,and the corresponding therapeutic principles and methods are proposed.‘Deficiency of spleen qi and accumulation of dampness,phlegm and blood stasis’is the core pathogenesis of early colorectal adenoma.The treatment should be‘strengthening spleen and promoting diuresis,resolving phlegm and removing blood stasis and resolving masses’,focusing on eliminating tumor and resolving masses,preventing recurrence and hyperplasia of adenoma.‘Deficiency of spleen qi,accumulation of dampness,phlegm and blood stasis and toxin’is the core pathogenesis of advanced colorectal adenoma canceration.The treatment should strengthen spleen and promote diuresis,eliminate phlegm and blood stasis,and resist cancer and detoxify.The emphasis is on anti-cancer and detoxify,and prevent and treat adenoma canceration.The main treatment methods include:Invigorating the spleen and replenishing qi,dissipating dampness and turbidity,dispelling phlegm,dispelling blood stasis and eliminating symptoms,anti-cancer and detoxification.
