Observation on effect of traditional Chinese and western medicine in treatment of coal worker’s pneumoconiosis with chronic obstructive pulmonary disease

摘要目的探讨清金化痰汤联合西医治疗煤工尘肺合并慢性阻塞性肺疾病的临床效果。方法回顾性分析2017年3月—2019年8月中国平煤神马集团职业病防治院诊治的92例煤工尘肺合并慢性阻塞性肺疾病患者临床资料,对照组采用克矽平、多索茶碱治疗,观察组在此基础上给予清金化痰汤,比较治疗前后两组第1秒用力呼气量(Forced expiratory volume in one second,FEV1)、用力肺活量(Forced vital capacity,FVC)、最大通气量(Maximal voluntary ventilation,MVV)、肺弥散功能(Pulmonary diffusion function,DLCO)、改良呼吸困难指数(Modified medical researchcouncil,mMRC)分级、慢阻肺测试(COPD assessment test,CAT)评分及临床疗效情况。结果治疗后FEV1、FVC、MVV、DLCO指标与mMRC分级、CAT评分改善幅度观察组均显著优于对照组,临床总有效率观察组为97.8%,对照组为82.6%,组间差异均有统计学意义(均P<0.05)。结论清金化痰汤联合西医治疗煤工尘肺合并慢性阻塞性肺疾病可更有效缓解患者呼吸困难症状,改善肺功能,提高治疗效果。 Objective To study the clinical effect of Qingjinhuatan Decoction combined with western medicine in treatment of coal worker’s pneumoconiosis with chronic obstructive pulmonary disease(COPD). Methods The clinical data of 92 cases of coal worker’s pneumoconiosis with COPD in the Occupational Disease Precaution Clinic of China Pingmei Shenma Group from March 2017 to August 2019 were analyzed retrospectively. The control group was treated with oxypovidine and doxofylline,and the observation group was given the Qingjinhuatan Decoction on this basis. The forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),maximum ventilation volume(MVV),pulmonary diffusion function(DLCO),modified dyspnea index(MMRC)classification,COPD assessment test(CAT) score and clinical effect were compared before and after the treatment in the two groups.Results After treatment,the improvement of FEV1,FVC,MVV,DLCO level,mMRC level and CAT score in observation group were significantly superior to those in control group. The total effective rate in observation group was 97.8% and that in control group was82.6%. The difference between the two groups had the statistically significant(all P<0.05). Conclusion Qingjinhuatan Decoction combined with western medicine in treatment of coal worker’s pneumoconiosis with COPD can more effectively relieve the symptoms of dyspnea,improve lung function and enhance the therapeutic effect.
