摘要
目的探讨防旋股骨近端髓内钉内固定术(PFNA)在股骨粗隆间骨折中的应用效果及其对骨代谢及炎症指标的影响。方法选取2018年8月至2020年5月我科室收治的80例股骨粗隆间骨折患者作为研究对象,按照随机数表法将其分为观察组(40例,PFNA治疗)与对照组[40例,动力髋加压螺钉内固定术(DHS)治疗]。比较两组的治疗效果。结果观察组的手术时间、骨折愈合时间及住院时间短于对照组,术中出血量、术中引流量少于对照组(P<0.05)。观察组的髋关节恢复优良率高于对照组(P<0.05)。术后3个月,两组的PⅠNP、25-(OH)D3及BGP水平均升高,CTX-1、TNF-α、IL-6及IL-10水平均降低,且观察组优于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论PFNA治疗股骨粗隆间骨折效果显著,其可缩短患者的手术时间、骨折愈合时间及住院时间,减少术中出血量及术中引流量,降低术后并发症发生率,改善患者骨代谢情况及炎症情况。
Objective To investigate the application effect of proximal femoral nail antirotation(PFNA)in the treatment of intertrochanteric fracture and its influences on bone metabolism and inflammatory indexes.Methods From August 2018 to May 2020,80 patients with intertrochanteric fracture admitted in our department were selected as the research objects and divided into observation group(40 cases,treated with PFNA)and control group[40 cases,treated with dynamic hip compression screw(DHS)]according to the random number table method.The treatment effect of the two groups was compared.Results The operation time,fracture healing time and hospital stay of the observation group were shorter than those of the control group,the intraoperative blood loss and intraoperative drainage volume were less than those of the control group(P<0.05).The excellent and good rate of hip joint recovery in the observation group was higher than that in the control group(P<0.05).At 3 months after operation,the levels of PⅠNP,25-(OH)D3 and BGP increased,and the levels of CTX-1,TNF-α,IL-6 and IL-10 decreased in both groups,and those in the observation group were better than the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion PFNA is effective in the treatment of intertrochanteric fracture,it can shorten the operation time,fracture healing time and hospital stay,reduce intraoperative blood loss,intraoperative drainage volume and the incidence of postoperative complications,and improve bone metabolism and inflammation of patients.
