摘要
目的探讨血清淀粉样蛋白A(SAA)、C-反应蛋白(CRP)和降钙素原(PCT)检测在儿童肺炎支原体肺炎诊断中的应用价值。方法回顾性分析该院住院诊治的患儿331例。根据患儿感染类型分成肺炎支原体肺炎组及细菌性肺炎组,其中肺炎支原体肺炎组177例,细菌性肺炎组154例,另选取30例该院体检健康儿童作为对照组。记录3组对象SAA、CRP和PCT检测结果,对其诊断效能进行评价。结果与健康对照组比较,细菌性肺炎组及肺炎支原体肺炎组SAA、CRP、PCT均明显升高,差异有统计学意义(P<0.01),细菌性肺炎组与肺炎支原体肺炎组比较,SAA、CRP及PCT差异有统计学意义(P<0.01)。SAA、CRP、PCT诊断细菌性肺炎时ROC曲线下面积(AUC)为0.98、0.85、0.895,诊断肺炎支原体肺炎时AUC为0.877、0.72、0.765。结论细菌性肺炎及肺炎支原体肺炎患儿急性期血清SAA、CRP、PCT水平均明显升高,相对于CRP和PCT,SAA对肺炎支原体肺炎更敏感,诊断价值更高。
Objective To investigate the clinical application value of serum amyloid A(SAA),C-reactive protein(CPR)and procalcitonin(PCT)in the diagnosis of mycoplasma pneumoniae pneumonia in children.Methods Three hundred and thirty-one cases of hospitalized children in our hospital were analyzed retrospectively.According to the type of infection,the children were divided into Mycoplasma pneumoniae pneumonia group and bacterial pneumonia group,including 177 cases of Mycoplasma pneumoniae pneumonia group and 154 cases of bacterial pneumonia group.Another 30 healthy children in our hospital were selected as the control group.The results of SAA,CRP and PCT were recorded and evaluated.Results Compared with the healthy control group,the level of SAA,CRP and PCT in the bacterial pneumonia group and mycoplasma pneumoniae pneumonia group were significantly increased,with the differences statistically significant(P<0.01).Compared with bacterial pneumonia group,the level of SAA,CRP and PCT in mycoplasma pneumoniae pneumonia group were statistically different(P<0.01).The area under the ROC curve(AUC)for SAA,CRP,and PCT in the diagnosis of bacterial pneumonia was 0.98,0.85,and 0.895,and the AUC for the diagnosis of mycoplasma pneumoniae pneumonia was 0.877,0.72,0.765.Conclusion In children with bacterial pneumonia or mycoplasma pneumoniae pneumonia,the levels of SAA,CRP,and PCT are significantly increased.Compared with CRP and PCT,SAA is more sensitive to mycoplasma pneumoniae pneumonia and has higher diagnostic value.
