未成熟粒细胞绝对计数及血清PCT、CRP水平联合检测在肺部细菌性感染诊断中的应用价值 被引量:6

摘要 目的:探讨未成熟粒细胞绝对计数(IG#)及血清降钙素原(PCT),C反应蛋白(CRP)水平联合检测在肺部细菌性感染诊断中的应用价值.方法:选取2018年5月~2020年8月我院肺部细菌性感染患者86例作为观察组,同期肺部病毒感染患者59例作为对照Ⅰ组,健康体检者59例作为对照Ⅱ组.对比三组及观察组不同病情程度患者IG#,血清PCT,CRP水平,偏回归系数分析肺部感染病情程度与IG#,血清PCT,CRP水平关系,受试者工作特征曲线(ROC)分析IG#,血清PCT,CRP水平联合诊断肺部细菌性感染的价值.结果:观察组IG#,血清PCT,CRP水平高于对照Ⅰ组,对照Ⅱ组(P<0.05);对照Ⅰ组IG#,血清CRP水平高于对照Ⅱ组(P<0.05);不同病情程度患者咳嗽,咳痰,气促,肺啰音,呼吸困难占比,IG#,血清PCT,CRP水平对比,差异有统计学意义(P<0.05);采用偏回归系数校正咳嗽,咳痰,气促,肺啰音,呼吸困难等混杂因素后,IG#,血清PCT,CRP水平仍与肺部感染病情程度有关(P<0.05);IG#,血清PCT,CRP联合诊断肺部细菌性感染的AUC值为0.822,大于任一指标单独检测.结论:IG#,血清PCT,CRP水平变化与肺部细菌性感染发生发展关系密切,联合诊断具有较高价值,对临床及时完善治疗方案具有指导意义. Objective:To investigate the value of the combined determination of immature granulocyte absolute count(Ig#)and serum procalcitonin(PCT)and C-reactive protein(CRP)levels in the diagnosis of pulmonary bacterial infection.Methods:Selection in May 2018 to August 2020 our hospital 86 cases of pulmonary bacterial infection patients as observation group,59 patients with lung infection for the same period as the contrast Ⅰ group,59 patients as control Ⅱ group health check-up.The levels of Ig#,serum PCT and CRP in patients with different degrees of disease in the three groups and the observation group were compared.Partial regression coefficient was used to analyze the relationship between the degree of pulmonary infection and Ig#,serum PCT and CRP levels.Receiver operating characteristic curve(ROC)was used to analyze the value of Ig#,serum PCT and CRP levels in the combined diagnosis of pulmonary bacterial infection.Results:IG#observation group,serum PCT and CRP level is higher than the contrast Ⅰ group,the comparison Ⅱ group(P<0.05);Control IG#Ⅰgroup,serum CRP level was higher than control Ⅱ group(P<0.05);The proportions of cough,sputum,shortness of breath,lung rales,dyspnea,Ig#,serum PCT and CRP levels in patients with different disease degrees were compared with statistical significance(P<0.05).After adjusting for confounding factors such as cough,expectoration,shortness of breath,lung rale,and dyspnea,the levels of Ig#,serum PCT and CRP were still correlated with the severity of pulmonary infection(P<0.05).The AUC value of Ig#,serum PCT and CRP in the combined diagnosis of pulmonary bacterial infection was 0.822,which was higher than that of either index alone.Con Clusi on:Changes in the levels of Ig#,serum PCT and CRP are closely related to the occurrence and development of pulmonary bacterial infection.Combined diagnosis is of high value and has guiding significance for the timely improvement of clinical treatment plan.
