未成熟粒细胞联合胱抑素C对脓毒症性急性肾损伤患者不良预后的评估价值 被引量:4

Evaluation of IG combined with sCys-C in adverse prognosis of patients with sepsis-induced AKI

摘要 目的评估未成熟粒细胞(IG)联合血清胱抑素C(sCys-C)对脓毒症性急性肾损伤患者不良预后的评估价值。方法选取本院收治的58例脓毒症性急性肾损伤(AKI)患者,根据患者28 d后是否生存分为生存组和死亡组,在入院时检测sCys-C、未成熟粒细胞绝对值和未成熟粒细胞百分比值。分析AKI不同分期时sCys-C、未成熟粒细胞绝对值和未成熟粒细胞百分比的差异。运用ROC曲线比较未成熟粒细胞绝对值、未成熟粒细胞百分比、sCys-C以及联合检测在评估脓毒性AKI预后的价值。结果不同AKI分期之间比较结果显示,3期sCys-C、未成熟粒细胞绝对值、未成熟粒细胞百分比显著高于1期和2期(P<0.05);但在2期中仅sCys-C显著高于1期(P=0.002)。3期死亡率显著高于1期和2期(P<0.05)。ROC分析结果显示,sCys-C、未成熟粒细胞绝对值和未成熟粒细胞百分比的曲线下面积(AUC)分别为0.943(0.841~0.982)、0.828(0.714~0.945)、0.895(0.818~0.979),sCys-C的AUC显著高于未成熟粒细胞绝对值和未成熟粒细胞百分比(P<0.001)。sCys-C联合未成熟粒细胞绝对值和未成熟粒细胞百分比检测的AUC高达0.982(0.950~0.998),较单一指标的AUC显著升高(P<0.001)。结论IG与sCys-C联合检测的敏感度和特异度最高,对脓毒症性AKI患者预后的评估具有很大价值。 Objective To evaluate the value of immature granulocyte(IG)combined with serum cystatin C(sCys-C)in assessing the adverse prognosis of acute kidney injury(AKI)patients with sepsis.Methods A total of 58 patients with septic acute kidney injury(AKI)admitted to our hospital were enrolled.The patients were divided into survival group and death group according to whether they survived after 28 days.The sCys-C,IG and IG%values were collected at admission.The differences in sCys-C,IG and IG%at different stages of AKI were analyzed.ROC curve was used to compare IG,IG%,sCys-C and combined detection in the evaluation of the prognosis of AKI in septic patients.Results The results of different stages of AKI showed that the sCys-C,IG and IG%of stage 3 were significantly higher than those of stage 1 and 2(P<0.05).However,only sCys-C was significantly higher in stage 2 than stage 1(P=0.002).In the mortality rate,stage 3 was significantly higher than stage 1(P=0.006)and stage IG%were 0.943(0.841-0.982),0.828(0.714-0.945),0.895(0.818-0.979).The AUC of sCys-C was significantly higher than IG and IG%(P<0.001).The combined test showed that the AUC of sCys-C combined with IG and IG%was significantly higher than that of single indicators(P<0.001).The AUC of sCys-C combined with the absolute value of immature granulocytes and the percentage of immature granulocytes was as high as 0.982(0.950-0.998),which was significantly higher than that of a single indicator(P<0.001).Conclusion The combined detection of IG and sCys-C has the highest sensitivity and specificity.It is of great value to evaluate the prognosis of patients with sepsis-induced AKI.
