CRP联合SAA对不同小儿肺炎的临床诊断价值 被引量:4

摘要 目的解析C反应蛋白(C-reactive protein,CRP)联合血清淀粉样蛋白A(serumamyloid protein,SAA)对小儿细菌性肺炎和支原体肺炎的临床诊断鉴别作用。方法纳入我院2018年10月—2020年4月间的400例肺炎患儿,其中细菌性肺炎200例(细菌组)、病毒性肺炎100例(病毒组)、支原体肺炎100例(支原体组),在选取同期50例健康体检小儿作为对照组,检测并比对SAA、CRP、WBC的水平,及细菌性肺炎、支原体肺炎的阳性检出情况。结果SAA水平比对:细菌组(328.41±79.39)mg/L>病毒组(124.55±40.52)mg/L>支原体组(17.50±8.04)mg/L>对照组(6.95±2.07)mg/L;CRP水平比对:细菌组(23.00±2.76)mg/L>支原体组(19.03±2.44)mg/L>病毒组(7.41±1.86)mg/L>对照组(3.30±1.15)mg/L;WBC水平比对:细菌组(15.44±2.89)×10^9/L>支原体组(11.75±2.40)×10^9/L>病毒组(7.90±2.20)×10^9/L>对照组(6.24±2.01)×10^9/L,P<0.05;细菌性肺炎检出率:SAA+CRP+WBC 93.50%>CRP 84.50%>SAA76.50%>WBC 72.50%;支原体肺炎检出率:SAA+CRP+WBC(82.00%)>SAA(52.00%)>CRP(48.00%)>WBC(39.00%),P<0.05。结论小儿肺炎临床诊断管理中,CRP与SAA联合检测的应用价值高,可提高检出率,利于辨别肺炎类型,为临床的治疗提供参考,推荐推广。 Objective To analyze the clinical diagnosis and differentiation of CRP(C-reactive protein)combined with SAA(serumamyloid protein,serum amyloid A)in children with bacterial pneumonia and mycoplasma pneumonia.Methods We included 400 children with pneumonia from October 2018 to April 2020 in our hospital,including 200 cases of bacterial pneumonia(bacteria group),100 cases of viral pneumonia(virus group),and 100 cases of mycoplasma pneumonia(mycoplasma group).In the same period,50 healthy children were selected as the control group to detect and compare the levels of SAA,CRP,WBC and the positive detection of bacterial pneumonia and mycoplasma pneumonia.Results SAA level comparison:bacterial group(328.41±79.39)mg/L>virus group(124.55±40.52)mg/L>mycoplasma group(17.50±8.04)mg/L>control group(6.95±2.07)mg/L;CRP level comparison:bacterial group(23.00±2.76)mg/L>mycoplasma group(19.03±2.44)mg/L>virus group(7.41±1.86)mg/L>control group(3.30±1.15)mg/L;WBC level comparison:bacterial group(15.44±2.89)×10^9/L>mycoplasma group(11.75±2.40)×10^9/L>virus group(7.90±2.20)×10^9/L>control group(6.24±2.01)×10^9/L,P<0.05;bacterial pneumonia detection rate:SAA+CRP+WBC 93.50%>CRP 84.50%>SAA76.50%>WBC 72.50%;mycoplasma pneumonia detection rate:SAA+CRP+WBC(82.00%)>SAA(52.00%)>CRP(48.00%)>WBC(39.00%),P<0.05.Conclusion In the clinical diagnosis and management of pediatric pneumonia,the combined application of CRP and SAA has high application value,which can increase the detection rate,help to identify the type of pneumonia,provide reference for clinical treatment,and recommend promotion.
