摘要
目的了解北京市感染性腹泻细菌病原谱的构成和流行特征。方法选取2017-2019年北京市16区哨点医院肠道门诊腹泻患者粪便样本,通过分离培养、血清分型及毒力基因检测方法对5种肠致泻性大肠埃希菌、沙门菌、副溶血性弧菌、志贺菌、O1/O139群霍乱弧菌和弯曲菌等多种肠道致病菌的检测和分析。结果北京市2017-2019年共监测腹泻患者18 182例,其中4 025例患者肠道致病菌检测阳性,检出率为22.14%。阳性菌株中,肠致泻性大肠埃希菌检出最多(38.20%),其次为沙门菌(20.80%)和副溶血性弧菌(15.92%)。肠道致病菌的人群分布主要集中在成年组,病原菌的检出具有明显的季节性,7—9月为高峰。结论 2017-2019年北京市感染性腹泻致病菌以肠致泻性大肠埃希菌为主,其次为沙门菌和副溶血性弧菌,病原菌在不同年龄组分布存在差异,且有明显的季节性流行特征。
Objective To understand the pathogen spectrum and epidemiological characteristics of infectious diarrhea caused by pathogenic bacteria in Beijing during 2017 and 2019. Methods Fecal samples were collected from patients with diarrhea in sentinel hospitals in Beijing from 2017 to 2019.Multiple enteric pathogens as diarrheagenic Escherichia coli(DEC), Salmonella, Vibrio parahaemolyticus, Shigella, O1/O139 Vibrio cholerae and Campylobacter were identified by culture isolation, serotyping and virulence gene detection. Results Out of total 18 182 stool samples, 4 025 were positive for enteric pathogenic bacteria with the overall positive rate of 22.14%.Among the isolated strains, DEC was the predominant pathogen(38.20%), followed by Salmonella(20.80%) and Vibrio parahaemolyticus(15.92%).Enteric pathogenic bacteria were mainly found in adults and in samples collected from July to September. Conclusions DEC is the most common pathogen causing infectious diarrhea in Beijing during 2017 and 2019, followed by Salmonella and Vibrio parahaemolyticus.The distribution of pathogens varies among different age groups and shows obvious seasonal patterns.
