摘要
目的:探讨阑尾切除术后患者在加速康复外科(ERAS)基础上联合应用黄藤通腑汤内服、中药穴位贴敷的康复效果。方法:急性阑尾炎在围手术期应用加速康复外科且行腹腔镜阑尾切除术的患者,共入组100例,随机分为对照组(常规治疗)和观察组(常规治疗基础上联合黄藤通腑汤内服、中药穴位贴敷)各50例,观察患者术后肠鸣音恢复时间、肛门首次排气时间、24 h视觉模拟评分法(VAS)疼痛评分、术后并发症发生率、体温恢复正常时间、住院天数等指标,并比较两组疗效差异。结果:术后24 h的VAS评分观察组显著低于对照组;术后并发症发生率观察组(13.00%)低于对照组(25.00%);住院时间观察组(5.60±1.80)d短于对照组的(7.60±2.40)d,体温恢复正常时间观察组(11.21±2.25)h短于对照组的(15.53±3.12)h,肛门首次排气时间观察组(18.80±5.60)h短于对照组的(23.67±6.93)h;治疗后比较,观察组患者CD3+、CD3+/CD4+、CD3+/CD8+均低于对照组。以上差异均有统计学意义(P<0.05)。结论:应用黄藤通腑汤内服及中药穴位敷贴,对腹腔镜阑尾切除术后患者可加速康复。
Objective:To explore the effect of the combined application of internal administration of Huangteng Tongfu Decoction(黄藤通腑汤)and Chinese materia medica acupoint application on the basis of accelerated rehabilitation on rehabilitation of patients with appendectomy.Methods:A total of 100 patients with acute appendicitis who underwent laparoscopic appendectomy and applied accelerated surgical rehabilitation during perioperative period were enrolled and randomly divided into the control group(conventional treatment)and the observation group(internal administration of Huangteng Tongfu Decoction and Chinese materia medica acupoint application on the basis of conventional treatment),with 50 patients in each group.The postoperative bowel sound recovery time,the first anal exhaust time,24 h visual analog scale(VAS)pain score,the incidence of postoperative complication,the time to return to normal body temperature,hospital stay and other indicators were observed,and the difference of curative efficacy between the two groups was compared.Results:The 24 h VAS score after operation in the observation group was significantly lower than that in the control group.The incidence of postoperative complications in the observation group(13.00%)was lower than that in the control group(25.00%).The length of hospital stay in the observation group(5.60±1.80)d was shorter than that in the control group(7.60±2.40)d,and the time to return to normal body temperature in the observation group(11.21±2.25)h was shorter than that in the control group(15.53±3.12)h.The first anal exhaust time in the observation group(18.80±5.60)h was shorter than that in the control group(23.67±6.93)h.After treatment,the levels of CD3+,CD3+/CD4+and CD3+/CD8+in the observation group were lower than those in the control group.All the above differences were statistically significant(P<0.05).Conclusion:The application of internal administration of Huangteng Tongfu Decoction combined with Chinese materia medica acupoint application can accelerate the posto
