摘要
目的探讨阿尔茨海默病(AD)患者认知域与脑皮层下结构体积的相关性。方法纳入来自南京大学医学院附属鼓楼医院神经内科的受试者65例,包括认知正常(NC)18例,轻度认知障碍(MCI)患者30例和AD患者17例。采用神经心理学评估认知域,高分辨磁共振评估皮层下结构。结果单因素方差分析提示,相对于NC组,AD组在记忆、语言、注意力、执行功能和视空间能力方面分值更低,MCI组在记忆和视空间能力方面分值更低。相对于MCI组,AD组在记忆和视空间能力方面分值更低。相对于NC组,AD组双侧丘脑、双侧海马、双侧杏仁核、双侧伏隔核和脑干结构体积明显缩小;MCI组双侧丘脑和左侧海马结构体积明显缩小。相对于MCI组,AD组双侧丘脑和双侧杏仁核结构体积明显缩小。偏相关分析提示记忆功能与左侧丘脑和双侧海马体积显著相关,语言功能与双侧丘脑和左侧海马体积显著相关,执行功能与左侧丘脑和右侧杏仁核体积显著相关,视空间功能与双侧丘脑和海马体积显著相关。一般线性回归分析表明左侧海马体积可能是记忆功能的预测指标,左侧丘脑体积则可能是语言功能、执行功能和视空间能力的预测指标。结论MCI和AD患者左侧海马体积可能是记忆功能的预测指标,左侧丘脑体积则可能是语言功能、执行功能和视空间能力的预测指标。
Objective tients with Alzheimer’s disease(AD).Methods Totally 65 cases from the Department of Neurology,Affiliated Drum Tower Hospital of Nanjing University Medical School,including 18 cases were cognitively normal(NC),30 cases were mild cognitive impairment(MCI)and 17 cases were AD.Neuropsychology was used to evaluate the cognitive domain and high resolution magnetic resonance was used to evaluate the sub-cortical structure.Results Compared with the NC group,the AD group had lower scores on memory,language,attention,executive function,and visuospatial ability,and the MCI group had lower scores on memory and visuospatial ability.Compared with the MCI group,the AD group had lower scores on memory and visuospatial ability.Compared with the NC group,the volume of bilateral thalamus,bilateral hippocampus,bilateral amygdala,bilateral nucleus accumbens and brain stem in AD group was significantly reduced.The volume of bilateral thalamus and left hippocampus in MCI group was significantly reduced.Compared with MCI group,the volume of bilateral thalamus and amygdala in AD group was significantly reduced.Partial correlation analysis suggested that memory function was significantly correlated with the volume of the left thalamus and the bilateral hippocampus,language function was significantly correlated with the volume of the left thalamus and the left hippocampus,executive function was significantly correlated with the volume of the left thalamus and the right amygdala,and visuospatial function was significantly correlated with the volume of the bilateral thalamus and the hippocampus.General linear regression analysis indicated that left hippocampal volume may be a predictor of memory function,while left thalamic volume may be a predictor of language function,executive function and visuospatial ability.Conclusion Left hippocampal volume may be a predictor of memory function in patients with MCI and AD,and left thalamus volume may be a predictor of language function,executive function and visuospatial ability.
