摘要
目的分析Pfirrmann分级法在经皮低温等离子射频消融术治疗腰椎间盘突出症(LDH)患者临床疗效中的评估价值。方法回顾性分析2017年5月~2019年5月于本院接受治疗的63例LDH患者临床资料,单椎间隙患者按照Pfirrmann腰椎间盘退变影响学分级标准:Ⅰ级0例、Ⅱ级15例、Ⅲ级19例、Ⅳ级29例、Ⅴ级0例(分为Ⅱ级组、Ⅲ级组、Ⅳ级组)。以改良Macnab疗效评价标准对患者治疗疗效进行评估,以疼痛数字评分(NRS)评估患者术前、术后7d、1个月、3个月及6个月疼痛情况,并记录所有患者术前及术后各时间点Oswestry功能障碍指数(ODI)。比较经皮低温等离子射频消融术治疗LDH的疗效,比较不同Pfirrmann分级NRS、ODI及临床疗效,记录所有患者术后并发症发生情况。结果患者术后1月、6月NRS评分均低于术前(P<0.05),术后1月与术后6月NRS评分比较无差异(P>0.05);术后1月、6月ODI评分均低于术前,术后6月ODI评分较术后1月低(P<0.05)。术后1个月优良率为88.89%,术后6个月优良率为80.95%,术后各时间段内优良率比较无差异(P>0.05)。3组组内比较术后各时间点NRS评分均低于术前(P<0.05);3组组间术前、术后7天NRS评分比较无差异(P>0.05);Ⅱ级组与Ⅲ级组各时间点NRS评分比较无差异(P>0.05);Ⅱ级组与Ⅳ级组、Ⅲ级组与Ⅳ级组术后7天NRS评分比较无差异(P>0.05),术后1月、3月及6月NRS评分均有差异(P<0.05)。3组组内比较术后各时间点ODI评分均低于术前(P<0.05);3组组间术前ODI评分比较无差异(P>0.05);两两比较,Ⅱ级组与Ⅲ级组各时间点评分比较无差异(P>0.05);Ⅱ级组与Ⅳ级组、Ⅲ级组与Ⅳ级组术后7天、术后1月、3月及6月NRS评分均有差异(P<0.05)。Ⅱ级组、Ⅲ级组与Ⅳ级组术后6月改良Macnab疗效评价比较有差异(P<0.05)。63例患者术后并发症发生率为11.11%。结论经皮低温经皮低温等离子射频消融术可显著改善LDH患者疼痛和功能障碍,疗效佳、
Objective To analyze the value of modified Pfirrmann grading in evaluating the clinical efficacy of percutaneous low-temperature plasma radiofrequency ablation in the treatment of patients with lumbar disc herniation syndrome(LDH).Methods The clinical data of 63 patients with LDH who were treated in our hospital from May 2017 to May 2019 were retrospectively analyzed.According to Pfirrmann lumbar disc degeneration influence classification standard,the patients with single intervertebral space were divided into gradeⅡGroup(n=0),gradeⅡGroup(n=15),gradeⅢgroup(n=19),gradeⅣgroup(n=29)and gradeⅤgroup(n=0).The modified macnab efficacy evaluation criteria was used to evaluate the therapeutic effect of the patients.The pain situation was evaluated by NRs before operation,7 days,1 month,3 months and 6 months after operation,and the Oswestry disability index(ODI)was recorded at each time point before and after operation.Objective to compare the efficacy of percutaneous low-temperature plasma radiofrequency ablation in the treatment of LDH,compare the NRs,ODI and clinical efficacy of different Pfirrmann grades,and record the postoperative complications of all patients.Results The NRS scores of 63 patients 1 month and 6 months after operation were lower than those before operation(P<0.05).There was no difference in NRS scores 1 month and 6 months after operation(P>0.05).The ODI scores of 63 patients 1 month and 6 months after operation were lower than those before operation,and the ODI score 6 months after operation was lower than that 1 month after operation(P<0.05).The excellent and good rate was 88.89%1 month after operation and was 80.95%6 months after operation.There was no difference in the excellent and good rate in each time period after operation(P>0.05).The NRS scores at each time point after operation in the three groups were all lower than those before operation(P<0.05).There was no difference in NRS scores between the three groups before operation and 7 days after operation(P>0.05).There was no differen
