摘要
目的探讨椎体氢质子 MR 波谱(~1H MRS)测量值与双能 X 线(DXA)及定量 CT(QCT)所测骨密度(BMD)值的相关性。方法 152名[30~80岁,平均(58±14)岁]女性志愿者分别进行以下检查:胸腰椎侧位(T4~L4)X 线平片,并采用 Genant 半定量法对其进行评阅。腰椎(12~4)用 DXA 及 QCT 进行 BMD 测量。腰椎常规行矢状面 T_2WI、T_1WI 和横轴面 T_2WI。MR 常规扫描结束后采用单体素点解析波谱(PRESS)序列对 L2~4椎体进行波谱采集,相应谱线采用 SAGE 7.0软件分析,并计算脂水比(LWR)、脂肪比(FF%)及水峰、脂峰线宽(LW)等相关椎体 MRS 定量指标。结果FF%、LWR 随年龄增加逐步增大;平均 LWR(2.71)与 DXA、QCT 所测平均 BMD 值(分别为1.038g/cm^2和104.2 mg/cm^3)具有相关性(r 值分别为-0.477、-0.769,P 值均<0.01)。水峰 LW随年龄增加呈逐渐变窄趋势,其同 DXA、QCT 测量的 BMD 具有相关性(r 值分别为0.439、0.712,P 值均<0.01)。脂峰线宽无明显变化趋势,且同 DXA、QCT 所测 BMD 无明显相关性(r 值分别为-0.183、-0.199,P 值均>0.05)。结论 MRS 可在常规腰椎 MR 基础上无创性评估椎体骨髓状况,进而来评估骨髓的变化对于骨量所带来的影响。
Objective To investigate the correlation between vertebral proton MRS with bone mineral density (BMD). Methods A total of 152 female volunteers ( mean age ( 58 ± 14) years, range 30-80 years) were recruited in this study. All subjects underwent lateral radiographs, BMD measurement by both dual X-ray absorptiometry (DXA) and quantitative computed tomography (QCT), as well as MRS examination. Lateral spine radiographs (T4-L4) were evaluated with the method of Genant semiquantitative assessment. BMD was obtained by both DXA and QCT at lumbar spine (L2-4). After routine MRI examination (Sagittal T2WI, T1WI and Axial T2WI), single voxel (SV) was chosen for the ^1H MRS acquisition at L2-4 vertebral body with PRESS sequence. MRS parameters were calculated by SAGE 7.0 software. Results The lipid to water ratio (LWR) and fat fraction ( FF% ) of vertebral ascended with aging. A negative correlation was found between LWR (2.71) and BMD (L2-4) results by both DXA ( 1. 038 g/cm^2 )and QCT( 104. 2 mg/cm^3 ) examination (r = -0. 477, -0. 769 respectively, all P 〈 0. 01 ). Meanwhile, the water line width (LW) decreased with aging. A positive correlation was demonstrated between water LW and BMD (L2-4) results by both DXA and QCT examination (r = 0.439, 0. 712 respectively, all P 〈 0. 01 ). There is no correlation between lipid LW and BMD both in DXA and QCT (r = -0. 183, -0. 199 respectively, all P 〉 0. 05 ). Conclusion Vertebral MRS could be considered as a noninvasive examination in addition to routine spine MRI to demonstrate vertebral bone marrow changes, which could be helpful for further evaluation of vertebral osteoporosis.
