摘要
目的探讨非霍奇金淋巴瘤(NHL)患者血清乳酸脱氢酶(LDH)、β2微球蛋白(β2-MG)及CD44水平与病情和预后的关系。方法选取2018年3月~2019年3月75例于四川大学华西医院进行治疗的NHL患者作为疾病组,根据病理分期不同,将其分为Ⅰ~Ⅱ期组36例和Ⅲ~Ⅳ期组39例,并选取同期收治的体检健康人75例作为对照组,记录NHL患者1年内的生存情况,并将疾病组分为死亡组16例和生存组59例,比较各组血清LDH、β2-MG、CD44水平差异,分析血清LDH、β2-MG、CD44水平与NHL患者病情及预后的关系。结果疾病组血清LDH、β2-MG、CD44水平均高于对照组(P<0.05)。Ⅰ~Ⅱ期NHL患者血清LDH、β2-MG、CD44水平低于Ⅲ~Ⅳ期的患者(P<0.05)。对NHL患者随访1年,NHL患者1年生存率为81.94%(59/72),死亡组血清LDH、β2-MG、CD44水平分比为(334.39±27.56)IU/L、(4.93±0.27)μg/mL、(337.64±12.56)ng/mL分别高于生存组的(278.98±39.14)IU/L、(4.42±0.68)μg/mL、(307.14±20.88)ng/mL。ROC曲线分析显示,根据曲线下面积(AUC)分别选取血清LDH、β2-MG、CD44的最佳诊断截点为292.57 IU/L、4.62μg/mL、313.47 ng/mL;NHL患者血清LDH、β2-MG、CD44三者联合预测患者预后的AUC、特异度、敏感度为0.892、90.6%、85.5%均高于单一指标检测。结论NHL患者血清LDH、β2-MG、CD44水平均出现升高,在不同临床分期的患者中差异明显,三者联合评估可以为NHL患者预后提供较为准确的预测。
Objective To explore the relationship between levels of serum lactate dehydrogenase(LDH),β2 microglobulin(β2-MG)and CD44 levels and conditions,prognosis in patients with non-hodgkin lymphoma(NHL).Methods Seventy-five NHL patients who were treated in the hospital from March 2018 to March 2019 were enrolled as disease group.According to the different pathological stages,they were divided into stageⅠ-Ⅱgroup(n=36)and stageⅢ-Ⅳgroup(n=39).Another 75 healthy controls who were admitted in the same period were enrolled as normal group.The survival situations of NHL patients within 1 year were recorded.Patients in disease group were divide into death group(n=16)and survival group(n=59).The differences in levels of serum LDH,β2-MG and CD44 were compared among all groups.And the relationship between the above indexes and conditions,prognosis in NHL patients was analyzed.Results The levels of serum LDH,β2-MG and CD44 in disease group were higher than those in normal group(P<0.05).The levels of serum LDH,β2-MG and CD44 in patients with NHL at stageⅠ-Ⅱwere lower than those at stageⅢ-Ⅳ(P<0.05).NHL patients were followed up for 1 year.The 1-year survival rate of NHL patients was 81.94%(59/72).The levels of serum LDH,β2-MG and CD44 in death group were(334.39±27.56)IU/L,(4.93±0.27)μg/mL and(337.64±12.56)ng/mL,which were higher than those in survival group[(278.98±39.14)IU/L,(4.42±0.68)μg/mL,(307.14±20.88)ng/mL].ROC curve analysis showed that according to the area under the curve(AUC),the best diagnostic cut-off values of serum LDH,β2-MG and CD44 were 292.57 IU/L,4.62μg/mL and 313.47 ng/mL,respectively.AUC,specificity and sensitivity of serum LDH combined withβ2-MG and CD44 for predicting the prognosis of NHL patients were 0.892,90.6%and 85.5%,which were higher than those of single index.Conclusion The levels of serum LDH,β2-MG and CD44 are increased in NHL patients.And there are significant differences among patients with different clinical stages.The combination of the three can provide more accur
