摘要
目的探讨子宫动脉栓塞术(UAE)联合甲氨蝶呤对剖宫产术后子宫瘢痕妊娠(CSP)的治疗价值及对β-绒毛膜促性腺激素(β-hCG)水平的影响。方法选取宁夏银川市第一人民医院2018年1月~2020年4月收治的122例CSP患者作为研究对象,依据随机数字表法分为联合组与药物组,每组各61例,其中药物组实施甲氨蝶呤治疗,联合组实施UAE联合甲氨蝶呤治疗。观察两组患者治疗前后的β-HCC水平、治疗后的手术指标及其并发症发生情况。结果治疗前两组患者的β-hCG水平比较,差异无统计学意义(P>0.05);治疗后,两组患者的β-hCG水平均低于治疗前,且联合组患者的β-hCG水平低于药物组,差异有统计学意义(P<0.05)。联合组患者的手术时间、住院时间短于药物组,术中出血量少于药物组,差异均有统计学意义(P<0.05)。联合组的并发症总发生率低于药物组,差异有统计学意义(P<0.05)。结论UAE联合甲氨蝶呤治疗CSP效果明确,缩短患者的手术时间、住院时间,减少术中出血量,且能够降低性腺激素水平,并发症少,临床安全可行。
Objective To explore the therapeutic value of uterine artery embolization(UAE)combined with Methotrexate on cesarean scar pregnancy(CSP)and the effect on the level human of β-chorionic gonadotropin(β-hCG).Methods A total of 122 cases CSP patients admitted to the the First People′s Hospital of Yinchuanfron January 2018 to April 2020 were selected as the research objects,they were classified into the combined group and the drug group according to the random number table method,each group had 61 cases,the drug group was treated with Methotrexate,combined group UAE combined with Methotrexate treatment.β-hCG level,operative indexes and complications of the two groups were observed.Results Before treatment,theβ-hCG levels of the two groups of patients were not statistically different(P>0.05);after treatment,the β-hCG levels of the two groups of patients were lower than those before treatment,and theβ-hCG levels of the combination group were low than those in the drug group,the differences were statistically significant(P<0.05).The operation time and hospital stay of the combined group were shorter than those of the drug group,and the intraoperative blood loss was less than that of the drug group,the differences were statistically significant(P<0.05).The total incidence of complications in the combined group was lower than that in the drug group,the difference was statistically significant(P<0.05).Conclusion UAE combined with methotrexate has a clear effect on CSP.It can shorten the operation time,hospital stay,reduce intraoperative blood loss,and reduce the level of gonadal hormones,with fewer complications,and is clinically safe and feasible.
