摘要
目的:分析乳腺癌手术患者DRG分组效果及住院费用影响因素,为推行DRG付费提供依据。方法:收集某三甲医院乳腺癌手术的住院患者信息,采用变异系数、总体方差减少系数评价DRG分组效果,通过多元逐步回归分析住院费用影响因素。结果:DRG分组CV值均小于1,RIV值为5.50%;住院天数、耗材费、药品费、手术方式、治疗效果和付费方式对住院费用有一定的影响。结论:乳腺癌手术患者DRG分组能够客观反映医疗资源消耗水平;医院应以DRG付费为契机,结合流程再造理念,规范诊疗流程,重点缩短住院天数,减少耗材费和药品费,从而降低住院费用。
Objective To analyze the DRG grouping effect of hospitalization expenses and its influencing factors of breast cancer patients,so as to provide basis for the implementation of DRG payment.Methods The information of inpatients undergoing breast cancer surgery in 3 A hospitals was collected,and the DRG grouping effect was evaluated by coefficient of variation and reduction coefficient of total variance,and the influencing factors of hospitalization expenses were analyzed by multiple stepwise regression.Results The CV value of DRG grouping was less than 1,and the RIV value was 5.50%.The hospitalization days,consumable material fee,drug fee,operation mode,treatment effect and payment method were associated with the hospitalization expenses.Conclusion The DRG grouping of breast cancer patients can objectively reflect the consumption level of medical resources,and hospitals should take DRG payment as an opportunity and combine the process reengineering theory to standardize the process of diagnosis and treatment,focus on shortening hospitalization days,reduce the consumables and drugs fee to reduce the hospitalization expenses.
