摘要
目的:观察基于中医润肤止痒思维的调护措施治疗特应性皮炎(AD)的临床研究。方法:将96例AD血虚风燥证患者随机分为观察组和对照组各48例。2组均予糠酸莫米松乳膏适量外用于皮损处,口服氯雷他定片,并给予生活环境指导、沐浴指导,保湿剂、润肤剂的使用指导和饮食指导等综合护理措施。观察组在润肤止痒的思维下给予由中医饮食调护、润肤止痒方外洗和外涂紫草婴儿软膏组成的调护措施。疗程均为3个月。治疗前、治疗3个月后评价特应性皮炎积分(SCORAD)和血虚风燥证评分;治疗后进行护理满意度评价。结果:治疗后,2组SCORAD的A、B、C各项评分及总分均较治疗前下降(P<0.01),观察组A、B、C各项评分及总分均低于对照组(P<0.01)。观察组血虚风燥证积分低于对照组(P<0.01),护理满意度高于对照组(P<0.05)。结论:在现代医学常规治疗和护理的基础上,在润肤止痒的思维下给予中医饮食调护、润肤止痒外方洗和紫草婴儿软膏的调护措施,能进一步减轻AD皮损程度,降低血虚风燥证评分,提高了护理满意度。
Objective:To observe the clinical study on regulating measures based on emollient and anti-pruritus thought in Chinese medicine for atopic dermatitis(AD).Methods:A total of 96 cases of AD patients with blood deficiency and winddryness syndrome were randomly divided into the observation group and the control group,48 cases in each group.Both groups were given mometasone furoate cream for skin lesions,oral loratadine tablets,and comprehensive nursing measures for living environment guidance,bathing guidance,moisturizer,emollient use and diet guidance.Under the thinking of moisturizing and relieving itching,the observation group was given TCM diet care,Runfu Zhiyang prescription external washing and comfrey baby ointment.Both groups were treated for three months.Before and after three months treatment,the scores of SCORing Atopic Dermatitis(SCORAD)and blood deficiency and wind-dryness syndrome were measured.After treatment,nursing satisfaction was evaluated.Results:After treatment,the scores of A,B and C as well as the total scores of SCORAD in the two groups were decreased when compared with those before treatment(P<0.01),and the above scores in the observation group were lower than those in the control group(P<0.01);the score of blood deficiency and wind-dryness syndrome in the observation group was lower than that in the control group(P<0.01);the nursing satisfaction in the observation group was higher than that in the control group(P<0.05).Conclusion:Based on routine western medicine treatment and nursing,the regulating measures including Chinese medicine diet nursing,external washing with Runfu Zhiyang prescription,and Zicao baby ointment under the emollient and anti-pruritus thought in Chinese medicine can further alleviate the skin lesions of AD,decrease the score of blood deficiency and wind-dryness syndrome,and improve nursing satisfaction.
