头颈部肿瘤放射性口腔黏膜炎患者营养状况及影响因素分析 被引量:19

Analysis of nutritional status and influencing factors in patients with head and neck tumor radiation oral mucositis

摘要 目的探讨头颈部肿瘤放射性口腔黏膜炎患者的营养状况及影响因素。方法选取2018年11月-2019年10月我院住院的头颈部肿瘤并发放射性口腔黏膜炎患者200例,进行主观整体评估量表(PG-SGA)及人口学的问卷调查,统计患者的PG-SGA得分情况,并进行营养状况的单因素和多因素分析。结果200例头颈部肿瘤放射性口腔黏膜炎患者PG-SGA总得分为(10.36±5.32)分。其中A级、B级、C级患者占比分别为8例(4.00%),39例(19.50%),153例(76.50%);头颈部肿瘤放射性口腔黏膜炎患者营养状况的多元回归分析显示,入院时BMI、TNM分期、口腔黏膜炎分级、味觉障碍、食欲减退、疼痛评分、摄入总热量、伴有抑郁8个因素是患者营养状况的独立影响因素(P<0.05)。结论头颈部肿瘤并发放射性口腔黏膜炎患者存在严重的营养不良,入院时BMI、TNM分期、口腔黏膜炎分级、味觉障碍、食欲减退、疼痛评分、摄入总热量、伴有抑郁8个因素是患者营养不良的独立影响因素,临床需进行早期的干预治疗和饮食指导,以提高患者的整体生存质量。 Objective To investigate the nutritional status and influencing factors of patients with radiation-induced oral mucositis of head and neck tumors.Methods From November 2018 to October 2019,200 patients with head and neck tumors and radiation oral mucositis who were hospitalized in our hospital were selected.The patient-generated subjective global assessment(PG-SGA)and demographic questionnaire were carried on,the PG-SGA score of patients was calculated,and single and multi-factor analysis of nutritional statusused were performed.Results The total score of PG-SGA in 200 cases of patients with head and neck tumor radiation oral mucositis was 10.36±5.32.Among them,the proportion of patients with grade A,B,and C was 8 cases(4.00%),39 cases(19.50%),and 153 cases(76.50%).Multiple regression analysis of nutritional status of patients with radiation-induced oral mucositis in head and neck tumors showed that 8 factors including BMI,TNM staging,oral mucositis grading,taste disorders,loss of appetite,pain score,total calorie intake,and depression were independent factors affecting patients’nutritional status at admission(P<0.05).Conclusion Patients with head and neck tumors with radiation-induced oral mucositis have severe malnutrition.BMI,TNM staging,oral mucositis grading,taste disorders,loss of appetite,pain score,total calorie intake,and depression are independent factors affecting patients’nutritional status at admission.Early interventions and dietary guidance are needed clinically to improve overall patient quality of life.
