摘要
目的探究增膜助孕汤对不孕症患者子宫内膜容受性以及雌孕激素受体的影响。方法82例不孕症患者根据随机数字表法分为对照组和观察组,各41例,对照组患者给予克罗米芬治疗,观察组患者在对照组基础上口服增膜助孕汤2个疗程(1个疗程为3个月经周期)。比较两组患者治疗前、治疗2个疗程后子宫内膜(厚度、类型)、子宫动脉血流参数子宫动脉血流阻力指数(RI)、搏动指数(PI)以及子宫内膜激素受体[雌激素受体(ER)、孕激素受体(PR)]表达水平差异,随访半年对比妊娠率。结果治疗2个疗程后较治疗前比较,两组患者子宫内膜厚度以及ER、PR蛋白水平均显著增加,RI、PI指数均显著降低,且同期观察组上述指标与对照组均有明显差异(P均<0.05)。两组患者治疗前后子宫内膜类型以A、B型为主,随访半年观察组妊娠率显著高于对照组(P<0.05)。结论增膜助孕汤可改善子宫内膜厚度、类型和血流情况,提高ER、PR表达水平,从而改善子宫内膜容受性,增加不孕症患者妊娠率。
Objective To explore the effects of Zengmo Zhuyun decoction on endometrial receptivity and estrogen and progesterone receptors in patients with infertility.Methods 82 cases of patients with infertility admitted to our hospital from January 2016 to June 2017 were divided into control group and observation group according to the random number table method,with 41 cases in each group.Control group was treated with clomiphene,and observation group was orally given Zengmo Zhuyun decoction for 2 courses(3 menstrual cycles as 1 course)on the basis of control group.The endometrium(thickness,type),uterine artery blood flow parameters[uterine arterial blood flow resistance index(RI),pulsatility index(PI)]and expression levels of endometrial hormones receptors[Estrogen receptor(ER),progesterone receptor(PR)]were compared between the two groups before treatment and after 2 courses of treatment.And at 6 months of follow-up,the pregnancy rate was compared.Results Compared with before treatment,the endometrial thickness and ER and PR protein levels were significantly increased in the two groups after 2 courses of treatment while the RI and PI indexes were significantly decreased,and the above indexes in observation group were significantly different from those in control group(all P<0.05).The types of endometrium in the two groups were mainly types A and B before and after treatment,and the pregnancy rate at 6 months of follow-up in observation group was significantly higher than that in control group(P<0.05).Conclusions Zengmo Zhuyun decoction can improve the thickness,type and blood flow of endometrium,and improve the expression levels of ER and PR,thus promoting the endometrial receptivity and increasing the pregnancy rate of patients with infertility.Therefore it has certain clinical application value in clinical practice.
