摘要
目的分析竹圈姜灸联合穴位敷贴治疗寒湿痹阻型类风湿关节炎的临床效果。方法选取2019年1月—2020年8月收治的寒湿痹阻型类风湿关节炎患者140例。通过随机数字表法将其分成对照组与研究组,各70例。对照组接受常规对症治疗,研究组则在常规对症治疗的同时,加用竹圈姜灸联合穴位敷贴治疗。观察比较2组患者临床疗效、关节症状(晨僵时间、关节肿痛)、数字模拟评分法(VAS)评分、血清免疫球蛋白水平和不良反应。结果研究组的临床总有效率为91.43%,明显高于对照组的74.29%,差异有统计学意义(P <0.05)。2组治疗后的晨僵时间、关节肿痛、VAS评分均明显优于治疗前(P <0.05);而且研究组治疗后的各项指标均明显优于对照组(P <0.05)。与治疗前相比,2组治疗后的各项血清免疫球蛋白(IgM、IgG、IgA)水平均明显降低(P <0.05);而且研究组治疗后的各项血清免疫球蛋白水平均明显低于对照组(P <0.05)。与对照组相比,研究组的口腔溃疡、过敏性皮疹、恶心呕吐、腹痛腹泻发生率均明显降低(P <0.05)。结论竹圈姜灸联合穴位敷贴治疗寒湿痹阻型类风湿关节炎患者取得令人满意的效果,不但能对其关节症状进行显著改善,提高机体免疫力,而且不良反应发生风险低,安全无不良作用,具有临床推广价值。
Objective To analyze the clinical effect of bamboo circle ginger moxibustion combined with acupoint application in the treatment of cold-dampness rheumatoid arthritis. Methods 140 patients with rheumatoid arthritis of cold-dampness obstruction type were selected from the rheumatology department of our hospital from January 2019 to August 2020. They were randomly divided into control group and study group,with 70 cases in each group. The control group received conventional symptomatic treatment,while the study group was treated with bamboo circle ginger moxibustion combined with acupoint application at the same time of conventional symptomatic treatment. The clinical efficacy,joint symptoms( morning stiffness time,joint swelling and pain,VAS score),serum immunoglobulin levels and adverse reactions were observed and compared between the two groups. Results The total clinical effective rate of the study group was 91. 43%,which was significantly higher than 74. 29% of the control group,and the difference was statistically significant( P < 0. 05). After treatment,morning stiffness time,joint swelling and pain,VAS score of both groups were significantly better than that before treatment( P < 0. 05). Moreover,all the indexes of the treatment group were significantly better than those of the control group( P < 0. 05). Compared with before treatment,the levels of various serum immunoglobulin( Ig M,Ig G,Ig A) in both groups after treatment were significantly decreased( P < 0. 05),in addition,the serum immunoglobulin levels of the treatment group were significantly lower than those of the control group( P < 0. 05).Compared with the control group,oral ulcers,allergic rashes,nausea and vomiting,the incidence of abdominal pain and diarrhea was significantly reduced( P < 0. 05). Conclusion Bamboo circle ginger moxibustion combined with acupoint application can achieve satisfactory results in the treatment of rheumatoid arthritis with cold-dampness obstruction type.
