摘要
类风湿关节炎(Rheumatoid Arthritis,RA)是一种以关节滑膜炎及对称性、破坏性的关节病变为主要特征的自身免疫性疾病,其病程长,致畸和致残率高,迄今尚无特效疗法。目前西医治疗RA常见药物有非甾体抗炎药、糖皮质激素、生物制剂等,但长期使用不良反应大,若治疗不及时,严重者将导致关节畸形和功能障碍,给患者的生活带来了极大痛苦。随着现代研究深入,中医治疗RA积累了丰富的临床经验,起到良好的效果。RA在中医归属于“痹病”范畴,常见证型有风寒湿痹、湿热痹阻、风湿热痹、痰瘀痹阻等,其中风寒湿痹型最为常见。中医各派医家治疗风寒湿痹型RA源远流长,留下了许多疗效显著的经方及治疗经验。中医治疗风寒湿痹型RA有其独特的治疗方法。常见中医外治法有温针灸、艾灸、针刺结合艾灸、火刃针、小针刀、中药蜡疗法、中药熏洗法、中药外敷、穴位贴敷等;中医内治法有经方与自拟方、中成药等。中医内治法与外治法相结合可发挥协同作用,提高临床疗效,更好地缓解临床症状、降低疾病活动度、减少不良反应。文章收集近十年中医治疗风寒湿痹型RA的文献进行汇总分析,探讨其独具特色的中医治法,为有效治疗本病提供理论基础,以期为临床实践提供借鉴和参考。
Rheumatoid arthritis(RA)is an autoimmune disease characterized by synovitis and symmetrical and destructive joint disease.It has a long course of disease,a high rate of teratogenesis and disability,and so far there is no specific treatment.At present,the common drugs used in Western medicine to treat RA include non-steroidal anti-inflammatory drugs,glucocorticoids,biological agents,etc,but long-term use has great adverse eactions,and if the treatment is not timely,severe cases will lead to joint deformity and dysfunction,which brings great pain to the life of patients.With the in-depth development of modern research,traditional Chinese medicine has accumulated rich clinical experience in the treatment of RA,which achieves a good effect.RA belongs to the category of“arthralgia”in traditional Chinese medicine.The common types are wind-cold dampness arthralgia,dampness-heat obstruction arthralgia,wind-dampness heat obstruction arthralgia,phlegm-stasis obstruction arthralgia,etc.Among them,wind-cold dampness arthralgia is the most common.Traditional Chinese medicine have a long history in treating wind-cold-dampness arthralgia RA,leaving many effective prescriptions and treatment experience.Traditional Chinese medicine has its unique treatment methods for wind-cold-dampness type RA.Common TCM external treatment methods include warm acupuncture,moxibustion,acupuncture combined with moxibustion,fire knife needle,small needle knife,TCM medicine wax therapy,TCM medicine fumigation and washing,TCM medicine external application,acupoint application,etc.TCM internal treatment methods include traditional prescription and self-modified prescription,Chinese patent medicine and so on.The combination of internal treatment and external treatment can play a synergistic role to improve clinical efficacy,better relieve clinical symptoms,reduce disease activity,and reduce adverse eactions.This paper collects and analyzes the literature on the treatment of wind-colddampness rheumatoid arthritis in the past ten years and discusses
