摘要
目的:探讨青中年卒中后癫痫发作的影响因素。方法:回顾性分析480例首次发病的青中年脑卒中患者,根据是否发作癫痫分为癫痫发作组和无癫痫发作组,对2组一般情况及发病因素进行单因素分析,并运用多因素Logistic回归分析导致卒中后癫痫发作的相关危险因素。结果:480例患者中出现癫痫发作患者42例。2组患者年龄、性别、文化程度、体重指数、同型半胱氨酸、高血压、高血脂、糖尿病、吸烟、饮酒发生率比较,差异无统计学意义(P>0.05);单因素分析结果显示,出血性卒中、病灶累及皮层、多脑叶病变、高NIHSS评分、心源性卒中患者癫痫发病率高,差异有统计学意义(P<0.05);2组患者mRS评分、是否有意识障碍发病率比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析显示,出血性卒中、皮层病变、多脑叶病灶、高NIHSS评分是脑卒中后癫痫的危险因素。结论:出血性卒中、皮层病变、病变范围累及多脑叶、高NIHSS评分的卒中患者更易发生癫痫。
Objective:To investigate the clinical characteristics and influencing factors of post-stroke seizures in young and middle-aged patients.Methods:480 young and middle-aged stroke patients with first onset were retrospectively analyzed.They were divided into epileptic seizure group and non epileptic seizure group according to whether they had seizures or not.Univariate analysis was performed on the general situation and risk factors of the two groups,and multivariate logistic regression was used to analyze the risk factors of post-stroke epilepsy.Results:there were 42 cases of epilepsy in 480 patients.There was no significant difference in age,gender,education level,body mass index,incidence of Hyperhomocysteine,hypertension,hyperlipidemia,diabetes,smoking and drinking between the two groups(P>0.05).Univariate analysis showed that incidence rate of seizures was higher in patients with hemorrhagic stroke,lesions involving cortex,multiple lobar lesions,high NIHSS score,and cardiac stroke(P<0.05).There was no significant difference in the mRS score and the incidence rate of consciousness disorder between the two groups(P>0.05).Multivariate logistic regression analysis showed that hemorrhagic stroke,cortical lesions,multiple lobar lesions and high NIHSS score were risk factors for epilepsy after stroke.Conclusion:Patients with hemorrhagic stroke,cortical lesions,multiple lobes lesions and high NIHSS score are more likely to have epilepsy.
